ADDENDUM TO AUBURN SENIOR ACTIVITY CENTER AGREEMENT
THIS ADDENDUM is made and entered into on the date last signed below by and
between Sea Mar Community Mental Health Centers ("Sea Mar"), a Washington charitable
organization, and the CITY Or, AUBURN, a Washington municipal corporation ("City"), as an
addendum to the Agreement between the parties for support to Latino seniors entered into on
January 9, 2017 ("Original Agreement").
RECITALS:
1. The parties entered into the Original Agreement for Sea Mar to provide an
advocate to assist Latino seniors who were using the Auburn Senior Center. Sea Mar provided
assistance one day per month.
2. The City has received funding that will allow this program to be expanded to one
day per week through the end of 2018.
AGREEMENT;
In consideration of the mutual promises contained in this Addendum, the parties agree as
follows:
1. Sea Mar will provide services one day per week from August 2, 2018 through
December 27, 2018. Beginning in January 2019, Sea Mar will return to providing service one day
per month.
2. The City will pay Sea Mar the sum of $4,225.00 for these additional services. Sea
Mar will invoice the City monthly.

3. REMAINING TERMS UNCHANGED: All other provisions of the Original
Agreement shall remain unchanged, and in full force and effect.

MEMORANDUM OF AGREEMENT
SEA MAR COMMUNITY HEALTH CENTERS
AND
AUBURN SENIOR ACTIVITY CENTER
This memorandum of agreement is between Sea Mar Community Health Centers Community
Living Connections Program and Auburn Senior Activity Center..
In recognition of:
I. The need to provide access to social and nutrition services.
2. The need to provide access to information, assistance and referrals to community
resources for low income Latino seniors as stipulated in the contract with the City of
Seattle Human Services Department Aging and Disability Services Sea Mar will
assign a senior advocate to provide outreach, interpretation and translation for Latino
seniors residing in the City of Auburn and surrounding areas during the lunch and
socialization activities at the Auburn Senior Activity Center; in addition the senior
advocate will also provide information, assistance and referrals other community
services.
Sea Mar's senior advocate will report to the Auburn Senior Activity Center the 3`d Thursday
of every month from 8:30 am to 2:30 pin. The day may change as deemed appropriate by both
parties, and the frequency may increase depending on funding availability.
Based on the above information Auburn Senior Activity Center and Sea Mar Community
Living Connections Program agree to the'following:
1. Auburn Activity Senior Center agrees to be open between the hours of 8:30 am and
2:30 pm on Thursday and/or at different day of the week as agreed. This time frame is
required to ensure safe arrival to and departure from the Auburn Senior Activity
Center of the Latino seniors attending the lunch program and socialization activities.
2. Sea Mar community Living Connections Program's staff will work in cooperation
with Auburn Senior Activity Center's staff and will also assist in setting and cleaning
Lip when necessary
3. Catholic Community Services will provide meals for the Latino seniors and reserves
the right to request a suggested donation of participants to help cover the cost of the
meal.
4, Auburn Senior Activity Center shall maintain a current Memorandum of Agreement
with Sea Mar Community Living Connections Program during the time of the
collaboration,
5. Tile term of this agreement shall be from January 19, 2017 to the day funding for the
Community Living Connections Program ends.
6. This memorandum of agreement may be terminated with thirty (30) days written
