HomeMy WebLinkAboutAgreement for Human Services with St Matthew Episcopal Church Olympia AMENDMENT No. 1 to the Community Matching Grant Agreement between City of Auburn and St.Matthew Episcopal Church,of the Diocese of Olympia Also known as San Mateo For good and valuable consideration, the parties agree to amend the above agreement in the following manner: ■ Section 1, "General Information" is amended to read as follows (revision is underlined), 1. General Information The Program is described by the following general information: a. Project Title: Improve Facility Infrastructure to Expand Outreach b. Description of the Project: This matching grant will allow repair of bathrooms in the parish hall so they are more accessible and capable of use by beneficiaries of services.The groups using this space include but are not limited to:two AA groups, two Alanon groups, Jubilee Center domestic abuse support and immigration and training programs, free medical and dental van, and community meals with the Auburn Food Bank. a. Term: The Project must be completed within one year of the signing of this contract. b. Area Served: 123 L St, Auburn, WA c. Grant Amount: up to $5,000 ■ Section 4, "Financial Administration" is amended to read as follows (revision is underlined), 4. Financial Administration a. The City has allocated $5.000 in its Neighborhood Program Annual Budget to complete the Project that is conditioned upon: i. A Grantee matching contribution of in-kind goods,services and/or cash donations in an amount equal or greater amount to$5,000, and incorporated herein by this reference. ii. The Grant Amount being used solely and strictly to complete the Project and the activities or services directly generated by, or associated with, the Project. iii. The preparation by the Project Coordinator, or his or her designee,of invoicing and proof of associated costs that is/are submitted to the City and Amendment No. 1 Page# 1 reviewed by Community Services staff for verification as to completion and form. b. Grantee shall pay all"out-of-pocket"expenses,and shall not be entitled to reimbursement from the City except for specific services, items, or activities described in Grantee's grant application and Exhibit A. • Section 5,"Agreement Term" is amended to read as follows (revision is underlined), 5. Agreement Term The term of this Agreement shall commence on the Effective Date and shall terminate on May 23, 2020, or upon completion of the Project Term, whichever is earlier, unless otherwise terminated pursuant to the provisions of this Agreement. • All other provisions of the Agreement and of any previous amendments remain in full force and effect. • The above amendment is intended to be severable, and if any provision hereof is illegal or invalid for any reason whatsoever, including lack of consideration, such illegality or invalidity shall not affect the validity of the remainder of this Agreement as originally executed by the Parties.
