My company may be liable for 75% of the balance remaining if services are disconnected or terminated during the initial term (Early Termination Fee). My company will also be billed for any Comcast Equipment that is not returned to a Comcast Service Center (like cable modem and television set-top boxes). If the GTC). My company will be responsible for outstanding amounts due for the Services through the actual date of termination. To view the GTC go to http://business.comcast.com/terms-conditions-smb If my company decides to reconnect its Business Class account, it will be assigned a new email address and Static IP (if applicable) and/or telephone number(s), as applicable. If my company is taking (porting) its telephone number(s) to another provider, it must contact the new provider to set up service. The new provider will contact Comcast to terminate the company's Business Class Voice Service (to ensure there will be no loss of service during the transfer). Pursuant to Comcast's records, ___________________________ Early Termination Fee liability (PRI Trunk, Business Voice Edge Accounts): Disconnect Request Form Pursuant to Comcast's records, ___________________________Early Termination Fee liability (Business Internet, Business Voice, Business Video Accounts): Transfer Existing Service Permanent Disconnect For Fiber based services: i. 100% of the monthly recurring charges with respect to months 1-12 of the Service Term; plus ii. 80% of the monthly recurring charges with respect to months 13-24 of the Service Term; plus iii. 65% of the monthly recurring charges with respect to months 25 through the end of the Service Term; plus iv. 100% of any remaining, unpaid Custom Installation Fees. outstanding amounts due for the Services through the actual date of termination. My company may also be liable for Termination Charges equal to the costs and expenses incurred by Comcast in installing or preparing to install the Service plus twenty percent (20%). Termination Charges shall be immediately due and payable upon cancellation or termination and shall be in addition to any and all accrued and unpaid charges for the Service rendered by Comcast through the date of cancellation or termination. Termination Charges will be calculated as follows: Termination Charges shall not apply to Service terminated by Customer as a result of Comcast's material and uncured breach in accordance with Article 5.2 of the General Terms and Conditions.
