Dear Lisa Moore:
The purpose of this letter is to confirm that T-Mobile West LLC decommissioned the antenna
facility located at the Lakeland Hills Water Reservoir Facility on January 15, 2016. The
decommission of the subject project is complete and in accordance with paragraph 17, stated
within the Site Lease Agreement dated December 4, 2003, T-Mobile has restored the facility
to original condition, normal wear and tear excluded.
