Requests approval of dental expenses in the amount of $358.00 for
services on June 24 and September 7, 2022.
2. LEOFF 1 Member #856535
Requests approval of dental expenses in the amount of $146.70 for
services on September 28, 2022.
3. LEOFF 1 Member #193495
Requests approval of medical expenses in the amount of $85.00 for
services on October 3, 2022. This member has not exceeded the
annual maximum amount for these services.
