1. LEOFF 1 Member #596469
Requests approval of Dental expenses in the amount of $4,629.00 for
services on March 13, 2023. This claim was tabled from the November
7th LEOFF Board meeting for additional information on medical necessity.

services received on October 30, 2023.

Requests approval of dental expenses in the amount of $1,500.00 for
services received on November 7, 2023.

Requests pre-approval of medical expenses for the purchase of new
hearing aids prior to the 5-year LEOFF Board requirement.
