Also present on he Zoom call and in person (City Hall, Conference Room 3) was Board Secretary Terry Mendoza, and HR Adminsitrative Assistant Samantha Norman.. Chair Jim Kelly and Deputy Mayor Rakes were excused. PUBLIC PARTICIPATION A. The Law Enforcement Officers and Firefighters Disability Board Special Meeting scheduled for Tuesday, June 10, 2025 at 10:00 a.m. will be held in person and virtually. Participants can submit written comments via mail, fax, or email. All written comments must be received prior to 5:00 p.m. on the day before the scheduled meeting and must be 350 words or less. Board Secretary Mendoza reported there was no correspondence received from the public. APPROVAL OF MINUTES A. Approval of the April 1 2025 LEOFF Board Meeting Minutes Motion by Member Laatsch to approve the minutes as published. Second by Councilmember Amer. MOTION PASSED UNANIMOUSLY. 3-0 ACTION ITEMS A. LEOFF Member #310533 i. Requests approval of dental expenses in the amount of $150.00 for services received on April 29, 2025. ii. Requests approval of dental expenses in the amount of $1,690.00 for services received on May 9, 2025. Motion by Member Laatsch to approve each claim as submitted. Second by Councilmember Amer. MOTION PASSED UNANIMOUSLY. 3-0 B. LEOFF Member #230947 Requests approval of dental expenses in the amount of $202.00 for services received on March 26, 2025. Motion by Councilmember Amer to approve the claim as submitted. Second by Member Laatsch. MOTION PASSED UNANIMOUSLY. 3-0 C. LEOFF Member #350627 Requests approval of medical expenses in the amount of $143.95 for durable medical equipment6 purchased on March 25, 2025. Motion by Member Laatsch to approve the claim as submitted. Second by Councilmember Amer. MOTION PASSED UNANIMOUSLY. 3-0 D. LEOFF Member #444135 i. Requests approval of medical expenses in the amount of $8,242.21 for home health care services received from March 14, 2025, through April 11, 2025. ii. Requests approval of medical expenses in the amount of $1,226.22 for home health care services received from April 15, 2025, through April 26, 2025. iii. Requests approval of medical expenses in the amount of $1,278.00 for home health care services received from April 28, 2025 through May 10, 2025. iv. Requests approval of medical expenses in the amount of $1,122.00 for home health care services received from May 12, 2025 through May 24, 2025. v. Requests pre -approval of medical expenses for the purchase of durable medical equipment. Motion by Member Laatsch to approve each claim as submitted. Second by Councilmember Amer. MOTION PASSED UNANIMOUSLY. 3-0 E. LEOFF Member #642873 Requests approval of dental expenses in the amount of 280.00 for services received on April 7, 2025. Motion by Member Laatsch to approve the claim as submitted. Second by Councilmember Amer. MOTION PASSED UNANIMOUSLY. 3-0 F. LEOFF Member #392099 i. Requests approval of vision expenses in the amount of $310.48 for services received on April 1, 2025. ii. Requests approval of medical expenses in the amount of $1,599.99 for hearing aids purchases on April 17, 2025. iii. Requests approval of dental expenses in the amount of $416.10 for services received on May 28, 2025. Motion by Councilmember Amer to table this claim pending a quorum since Chair Pro Tem Petersen would need to abstain from these items. Second by Member Laatsch. MOTION PASSED UNANIMOUSLY. 3-0 ADJOURNMENT There being no further business to come before the board, the meeting was adjourned at 10:09 a.m. APPROVED this 1st day of July 2025.
