CALL TO ORDER Chair Jim Kelly called the meeting to order at 10:00 AM on Monday, February 4, 2002, in Room 1 located at Auburn City Hall, 25 West Main Street, Auburn, WA. Chair Kelly and the following Board members were present: Chair Pro Tem Bud Larson and Member Stacey Brothers. Member Russ Vandver arrived at 10:03. Member Gene Cerino was excused. Board Secretary Danielle Daskam was also present. I1. APPROVAL OF MINUTES A. Minutes of the January 7, 2002 LEOFF Board Meeting Motion by Chair Pro Tern Larson, second by Member Brothers to approve the minutes as distributed. MOTION CARRIED UNANIMOUSLY. 3-0. III. UNFINISHED BUSINESS A. Pending Disability Leave/Retirement Applicationc There was no pending disability leave/retirement application. B. Claims 1. Richard Huseb¥ a. Request for Board consideration on Mr. Huseby's request to purchase hearing aids from other than the Board designated hearing aid provider. On January 7, 2002, the Board tabled Mr. Huseby's request pending receipt of equivalent cost information from the Board designated hearing aid provider, Costco. No new information was received. Motion by Chair Pro Tern Larson, second by Member Brothers to table Mr. Huseby's claim until the next meeting and receipt of Costco's written response for equivalent hearing aid costs. MOTION CARRIED UNANIMOUSLY. 4-0. 2. Bruce Caldwell a. Regence Explanation of Benefits Claim No. 0130432051800 processed on November 21, 2001 for services received on October 29, 2001 in the amount of $150.00. b. Regence Explanation of Benefits Claim No. 0131032080500 processed on November 21, 2001 for services received on November 1, 2001 in the amount of $150.00. c. Regence Explanation of Benefits Claim No. 0131634112600 processed on November 21, 2001 for services received on November 5, 2001 in the amount of $300.00. On January 7, 2002, the LEOFF Board tabled Mr. Caldwell's claims pending the receipt of additional information explaining the medical procedure or care received. No new information was received. Chair Kelly reviewed the pending claims. Chair Kelly also reported that he received an e-mail from Mr. Caldwell regarding the Board's correspondence requesting additional information relating to his claims. Chair Kelly explained to Mr. Caldwell by return e-mail that the Board requires specific information as to the procedure and its medical necessity. Motion by Member Brothers, second by Member Vandver to table Mr. Caldwell's claims until the next meeting pending the receipt of the additional information requested. Dave Schuur a. Request for reimbursement of lense and frames expenses in the amount of $15.43. On January 7, 2002, the LEOFF Board tabled Mr. Schuur's claim pending the receipt of a vision care worksheet. The Board reviewed a vision care worksheet submitted by Mr. Schuur. The Board noted the worksheet did not equal the amount on the provider receipt. The Board also discussed whether the claim included the co-pay amount of $10.00. Chair Kelly noted that according to the worksheet the total cost for frames and lenses is $335.00. The claim appears to exceed the reimbursement limit. Motion by Member Brothers, second by Chair Pro Tern Larson to table Mr. Schuur's claim for an additional month and request clarification of the claim from Mr. Schuur. MOTION CARRIED UNANIMOUSLY. 4-0. IV. NEW BUSINESS A. Disability Leave/Retirement Application,~ There was no new application for disability leave/retirement. B. Appeals No request for reconsideration was filed. C. Medical Claims 1. Alan Kosko a. Request for pre-approval of additional procedures supplemental to the oral surgery, which was approved by the Board on January 7, 2002. Chair Kelly recalled that on January 7, 2002, the Board approved $10,028 for the oral surgery. According to the letter, there are additional expenses related to the surgery. Board members expressed concern that the letter and attachments were unclear as to the additional procedure and its expense. Motion by Chair Pro Tern Larson, second by Member Brothers to table Mr. Kosko's request for additional expenses pending clarification of his claim for oral surgery. MOTION CARRIED UNANIMOUSLY. 4-0. 2. Bruce Caldwell a. Regence Explanation of Benefits Claim No. 0133232601800 processed on January 9, 2002 for services received on November 26, 2001 and Regence Explanation of Benefits Claim No. 0133032381300 processed on January 9, 2002 for services received on November 19, 2001 in the total amount of $600.00. b. Regence Explanation of Benefits Claim No. 0134732979700 processed on December 26, 2001 for services received on November 29, 2001 in the amount of $150.00. c. Regence Explanation of Benefits Claim No. 0131832004600 processed on January 23, 2002 for services received on November 12, 2001 in the amount of $150.00. Chair Kelly reviewed Mr. Caldwell's claims. He noted that the items identified as surgery on Regence Claim Nos. 0134732979700 and 0131832004600 were paid by the insurance, but the same items identified as surgery on Regence Claim Nos. 0133232601800 and 0133032381300 were not covered by insurance according to Regence Explanation of Benefits. Board members questioned the inconsistency in insurance coverage. LEOFF Board Minutes February 4, 2002 Page 4 City of Auburn Law Enforcement Officers and Firefighters Disability Board Monday, February 4, 2002 - 10:00 AM - Conference Room I Motion by Chair Pro Tern Larson, second by Member Brothers to table Mr. Caldwell's claims pending additional information from Mr. Caldwell as to the medical necessity of the medical services and clarification from the Association of Washington Cities Benefits Specialist and the insurance company as to the reason for Regence's coverage of some of the surgery charges while others are not covered by insurance. MOTION CARRIED UNANIMOUSLY. 4-0. D. Correspondence 1. Chair Kelly acknowledged receipt of a copy of the 2001 Summary of Annual Financial Report for the Year Ended June 30, 2001, from the Washington State Department of Retirement Systems. Copies were provided to the Board members for information only. V. ADJOURNMENT There being no further business to come before the Board, the meeting adjourned at 10:28 AM.
