MINUTES
CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Council
Conference Room located at Auburn City Hall, 25 West Main Street in Auburn.
Chair Kelly and the following Board members were present: Chair Pro Tern Bud
Larson (member at large), Member Lynn Norman, Member Virginia Haugen and
Member Bill Petersen. Board Secretary Danielle Daskam was also present.
II. APPROVAL OF MINUTES
A. Minutes of the August 5, 2008 LEOFF Board Meeting*
Motion by Chair Pro Tern Larson, second by Member Petersen to approve
the August 5, 2008 minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 5-0.
III. UNFINISHED BUSINESS
There was no unfinished business.
IV. NEW BUSINESS
A. Disability Leave/Retirement Applications
No new application for disability leave/retirement was received.
B. Appeals/Requests for Reconsideration
No request for reconsideration was received.
C. Medical/HearingNision Claims
Bruce Caldwell
Request for reimbursement of medical expenses in the amount of
$21.22, Regence Claim #0817720821400 for services received on
April 11, 2008.
Motion by Member Petersen, second by Chair Pro Tern Larson to
approve Mr. Caldwell's claim in the total amount of $21.22.
MOTION CARRIED UNANIMOUSLY. 5-0.

Request for pre-approval of dental expenses in the amount of
$1,480.10.
On July 1, 2008, the LEOFF Board advised Mr. Huseby that policy
8.02(D) provides for reimbursement of dental related expenses up
to $1,200 annually; any request for reimbursement of expenses
above the annual limit of $1,200 must be proven to be medically
necessary and accompanied by supporting documentation
evidencing the medical necessity.
Mr. Huseby submitted a letter from Stuart Rich, DDS, PS. In
support of his claim.
Motion by Member Petersen, second by Chair Pro Tern Larson to
table the claim pending the receipt of a physician's statement
supporting the medical necessity of Mr. Huseby's dental work.
MOTION CARRIED UNANIMOUSLY. 5-0.
3. Russ Vandver
Request for reimbursement of medical expenses in the amount of
$68.41, Regence Claim #08168170023100 for services received
on February 12, 2008.
Motion by Member Petersen, second by Chair Pro Tern Larson to
approve Mr. Vandver's claim in the amount of $68.41.
MOTION CARRIED UNANIMOUSLY. 5-0.
4. John Baker
Request for reimbursement of vision expenses in the amount of
$169.97 for services received on August 11, 2008.
Mr. Baker has not received reimbursement for vision expenses in
the last year.
Motion by Member Petersen, second by Chair Pro Tern Larson to
approve Mr. Baker's claim in the amount of $169.97.
MOTION CARRIED UNANIMOUSLY. 5-0.

Request for reimbursement of medical expenses in the amount of
$1,275.00, Regence Claim #0817520474100 for services received
on June 16, 2008.
Motion by Chair Pro Tern Larson, second by Member Petersen to
approve Mr. Monnett's claim in the amount of $1,275.00.
MOTION CARRIED UNANIMOUSLY. 5-0.
6. Robert S. Phillips
Request for reimbursement of vision expenses in the amount of
$219.97 for services received on August 6, 2008.
Mr. Phillips has not received reimbursement for vision expenses in
the last year.
Motion by Member Petersen, second by Chair Pro Tern Larson to
approve Mr. Phillips' claim in the amount of $219.97.
MOTION CARRIED UNANIMOUSLY. 5-0.
7. William Pitzl
Request for reimbursement of medical expenses in the amount of
$230.09, Regence Claim #0819032624600 for services received
on June 13, 2008.
Motion by Chair Pro Tern Larson, second by Member Petersen to
approve Mr. Pitzl's claim in the amount of $230.09.
MOTION CARRIED UNANIMOUSLY. 5-0.
8. William Petersen
Mr. Petersen's claim was added to the agenda.
Request for reimbursement of vision exam expenses in the amount
of $44.00, Regence Explanation of Benefits Claim
#0821432242600 processed on August 27, 2008 for services
received on July 8, 2008.
Motion by Chair Pro Tern Larson, second by Member Norman to
approve Mr. Petersen's claim in the amount of $44.00.
MOTION CARRIED. 4-0. Member Petersen abstained from
voting.
