MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Council
Conference Room located at Auburn City Hall, 25 West Main Street, Auburn.
Chair Kelly and tRe following members were present: Vice Chair Bud Larson,
Member Bill Peloza, and Member Roger Thordarson. Member Russ Vandver
arrived at 10:02 a.m. Also present during the meeting were Board Attorney
Joseph Beck and Board Secretary Danielle Daskam.
II. APPROVAL OF MINUTES
A. Minutes of the September 7. 2004 LEOFF Board MeetinQ
Motion by Chair Pro Tem Larson, second by Member Thordarson to
approve the September 7,2004 LEOFF Board minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 4-0.
III. UNFINISHED BUSINESS
A. PendinQ Disabilitv Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Medical/HearinQNision Claims
1. Robert Reno
a. Request for reimbursement of medical expenses in the
amount of $494.50 for medical services received in May and
June 2004. On August 3 and September 7, 2004, the
LEOFF Board tabled Mr. Reno's claim pending the receipt of
the applicable insurance explanation of benefits statement.
No new information was been received.
Motion by Chair Pro Tem Larson, second by Member
Thordarson to table Mr. Reno's claim until the next regular
meeting.
MOTION CARRIED UNANIMOUSLY. 5-0.

A. Disabilitv Leave/Retirement Applications
There was no new disability leave/retirement application.
B. Appeals
No appeal was filed.
C. Medical/HearinQNision Claims
1. ROQer H. Lee
Request for reimbursement of medical/pharmacy expenses in the
amount of $108.21.
Motion by Chair Pro Tem Larson, second by Member Thordarson
to return Mr. Lee's claim and request clarification of his claim
including identification of the items requested for reimbursement.
MOTION CARRIED UNANIMOUSLY. 5-0.
D. Correspondence
1. Correspondence received from Ken Crowder of the Snohomish
County Disability Board regarding History/Update Snohomish
County Disability Board Lawsuit.
Chair Kelly acknowledged receipt of the correspondence from Ken
Crowder of the Snohomish County Disability Board. The
correspondence from Mr. Crowder was distributed for informational
purposes only..
E. Discussion
1. According to LEOFF Board Policy and Procedure, 1.05, Elections,
in October of each even numbered year, the LEOFF Board
Secretary is to prepare and mail forms for nomination of the
firefighter representative to each active and retired firefighter
member. Fire Representative Russ Vandver's term expires on
December 31,2004.
Board members reviewed the election schedule. Nomination forms
will be October 15, 2004. No action was required by the Board.
