CALL TO ORDER Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Council Conference Room located at Auburn City Hall, 25 West Main Street, Auburn. Chair Kelly and the following Board members were present: Chair Kelly, Chair Pro Tern Bud Larson, Member Bill Peloza, Member Roger Thordarson and Member Russ Vandver. Also present during the meeting were Board Attorney Joseph Beck and Board Secretary Danielle Daskam. II. APPROVAL OF MINUTES A. Minutes of the November 2. 2004 LEOFF Board Meetina Motion by Chair Pro Tem Larson, second by Member Vandver to approve the November 2, 2004 minutes as distributed. MOTION CARRIED UNANIMOUSLY. 5-0. III. UNFINISHED BUSINESS A. Pendina Disabilitv Leave/Retirement Applications There was no pending disability leave/retirement application. B. Medical/HearinaNision Claims There was no pending claim. IV. NEW BUSINESS A. Disabilitv Leave/Retirement Applications There was no new disability leave/retirement application. B. Appeals/Reauests for Reconsideration 1. Robert Reno a. Request for reimbursement of medical expenses in the amount of $494.50 for medical services received in May and June 2004. On November 7, the LEOFF Board denied Mr. Reno's claim because no insurance explanation of benefits statement was submitted with the claim and there was a lack of detailed accounting for the amount requested for reimbursement. Miller, ND in support of his claim. Board Secretary Daskam noted that there were duplicate itemizations for services received on May 26, 2004, and the duplicate charges are the reason for the difference in the claim amount and the itemized amount shown above. Motion by Member Peloza, second by Member Vandver to approve the claim in the amount of $378.75. In response to questions from Board members, Attorney Beck explained that naturopathic physicians are licensed under State statute. The Board policy provides that covered medical services include drugs and medicines upon a physician's prescription. Attorney Beck also noted that the medical services defined in Policy 8.03 are minimum services to be provided. C. Medical/HearinQNision Claims 1. Murrav M. Board Request for reimbursement of vision expenses in the amount of $185.97. (Mr. Board has not received vision benefits within the last twelve months.) Motion by Chair Pro Tern Larson, second by Member Thordardson to approve Mr. Board's claim in the amount of $185.97. MOTION CARRIED UNANIMOUSLY. 5-0. 2. Helen J. Bowden Request for reimbursement of vision expenses in the amount of $99.97. (Ms. Bowden has not received vision benefits within the last twelve months.) Motion by Chair Pro Tem Larson, second by Member Vandver to approve Ms. Bowden's claim in the amount of $99.97. MOTION CARRIED UNANIMOUSLY. 5-0. 3. Michael J. Darbv Request for reimbursement of vision expenses in the amount of $276.00. (Mr. Darby has not received vision benefits within the last twelve months.) Motion by Member Peloza, second by member Vandver to approve $200.00 of Mr. Darby's claim in accordance with Board policy. MOTION CARRIED UNANIMOUSLY. 5-0. 4. Gerrv Fuller Request for reimbursement of vision expenses in the amount of $166.00. (Mr. Fuller has not received vision benefits within the last twelve months.) Motion by Member Vandver, second by Chair Pro Tem Larson to approve Mr. Fuller's claim in the amount of $166.00. MOTION CARRIED UNANIMOUSLY. 5-0. Wickstrom Regence Explanation of Benefits Statement Claim #0419495495501 processed on August 16, 2004 in the amount of $51.92 for medical services received on May 4, 2004. Motion by Chair Pro Tem Larson, second by Member Thordarson to approve Mr. Wickstrom's claim in the amount of $51.92. MOTION CARRIED UNANIMOUSLY. 5-0. 6. Robert Almv Request for reimbursement of medical expenses incurred on March 29,2004 in the amount of $11.80. Motion by Chair Pro Tem Larson, second by Member Peloza to approve Mr. Almy's claim in the amount of $11.80 subject to the receipt of the Regence Explanation of Benefits Statement. MOTION CARRIED UNANIMOUSLY. 5-0. 7. Richard Husebv Request for reimbursement of hearing aid repairs in the amount of $372.50. Motion by Member Peloza, second by Member Vandver to approve Mr. Huseby's claim in the amount of $370.00. Board members noted that $2.50 of the claim was not itemized. MOTION CARRIED UNANIMOUSLY. 5-0. D. Discussion 1. Membership Chair Kelly noted that Chair Pro Tem Larson's term as member at large will expire on December 31, 2004. Chair Pro Tem Larson expressed a desire to continue as the Board's member at large representative. Motion by Member Vandver, second by Member Peloza to appoint Bud Larson as the Board's member at large for a term beginning January 1, 2005 through December 31,2006. MOTION CARRIED UNANIMOUSLY. 5-0.
