CALL TO ORDER Chair Kelly called the meeting to order at 10:00 AM on Monday, July 1, 2002, in Room 1 located at Auburn City Hall, 25 West Main Street, Auburn, WA. Chair Kelly and the following Board members were present: Member Stacey Brothers, Member Gene Cerino and Member Russ Vandver. Chair Pro Tern Bud Larson was excused from the meeting. Others present included: Associate City Attorney Karen Gulliver, Human Resources Director Brenda Heineman, and Board Secretary Danielle Daskam. Guests present included: Mr. W. Terry Wood of Lehmann/VVood and Associates, and Mr. Bill Kyle. II. APPROVAL OF MINUTES A. Minutes of the May 6, 2002 LEOFF Board Meetin.q Motion by Member Cerino, second by Member Vandver to approve the May 6, 2002 minutes. MOTION CARRIED UNANIMOUSLY. 4-0. B. Minutes of the June 3~ 2002 LEOFF Board Meetin.q Motion by Member Vandver, second by Member Brothers to approve the June 3, 2002 minutes. MOTION CARRIED. 3°0. Member Cerino abstained. III. UNFINISHED BUSINESS A. Pending Disability Leave/Retirement Applications 1. Stan Laatsch Effective Date of Disability Leave: April 1, 2002 Last Day of Service: March 31, 2002 On April 1, 2002, the LEOFF Board approved an application for disability leave/retirement for Stan Laatsch effective April 1, 2002. The last physician's report was dated May 29, 2002 and was accepted by the Board at the June 3, 2002 meeting. (The letter from Dr. Becker was distributed during the meeting.) Motion by Member Cerino, second by Member Vandver to continue Mr. Laatsch's disability leave for an additional month or until cessation of disability, whichever occurs first. MOTION CARRIED UNANIMOUSLY. 4-0. B. Claims 1. Richard Huseby Regence Explanation of Benefits Claim No. 0205010749701 processed on March 20, 2002 with reference to Medicare Explanation of Benefits Statement No. 0127550209700 for services received on August 31, 2001 in the amount of $56.70. On May 6 and June 3, 2002, the LEOFF Board tabled Mr. Huseby's claim pending the receipt of additional information including the Medicare Explanation of Benefits Statement and proof of medical necessity. No new information was received. Motion by Member Brothers, second by Member Cerino to table Mr. Huseby's claim until the next meeting pending receipt of the requested information. MOTION CARRIED UNANIMOUSLY. 4-0. IV. NEW BUSINESS A. Disability Leave/Retirement Applications 1. Rudy Peden Effective Date of Disability: June 6, 2002 Last Day of Service: June 5, 2002 On June 6, 2002, the LEOFF Board Chair received and tentatively approved an application for Disability Leave or Retirement effective June 6, 2002 from Battalion Chief Rudy Peden. Wright, MD and Jayashree Srinivasan, MD dated May 20, 2002. Motion by Member Cerino, second by Member Vandver to confirm the Chair's action and approve the disability leave for Mr. Peden effective June 6, 2002, with last date of service on June 5, 2002. MOTION CARRIED UNANIMOUSLY. 4-0. B. Appeals No request for reconsideration was filed. C. Medical Claims 1. Edward Sluys Request for reimbursement of lens and frames expenses dated June 17, 2002 as follows: Bifocal Lenses: $110.00 Lens Tint: $ 30.00 Frames: $ 73.78 LEOFF Board Policy 9.02(A)(2) provides for reimbursement as follows: Bifocal Lenses: $ 60.00 Frames $ 90.00 Motion by Member Cerino, second by Member Vandver to approve reimbursement of $73.78 for frames and $60.00 for bifocal lenses for a total reimbursement of $133.78. MOTION CARRIED UNANIMOUSLY. 4-0. 2. Ed Henderson Request for pre-approval of oral surgery expenses. Board members reviewed Mr. Henderson's letter dated June 5, 2002 and the accompanying letter from Auburn Valley Endodontics dated May 30, 2002. LEOFF Board Minutes July 1, 2002 Page 3 City of Auburn Law Enforcement Officers and Fireflghters Disability Board Monday, July 1, 2002 - 10:00 AM - Conference Room 1 Motion by Member Brothers, second by Member Cerino to table Mr. Henderson's request pending receipt of the following information: 1) medical necessity of the treatment; 2) any alternative treatment and cost associated with the alternative treatment; and 3) and the causative factor of Mr. Henderson's condition. MOTION CARRIED UNANIMOUSLY. 4-0. D. Discussion 1. Dental Insurance Coverage Human Resources Director Heineman reported that approximately 57 LEOFF 1 members purchase preventative dental coverage through the Association of Washington Cities. The retiree dental insurance plan provides a maximum annual benefit of $1,000. She explained that two to three years ago, the Association of Washington Cities and the dental insurance companies opened up enrollment to retirees. Those who did not elect to enroll at that time or at time of retirement are no longer eligible to enroll. The monthly premium for the retiree is $47.80 and for employee and spouse is $95.59. Chair Kelly noted that dental coverage is not a mandatory benefit for LEOFF 1 members. Ms. Heineman agreed to provide the Board with a copy of retiree dental plan. 2. Long-term Care Insurance Coverage Human Resources Director Heineman introduced Mr. Wood of Lehman/Wood and Mr. Kyle. Mr. Wood presented the Board members with information on the Allianz long-term care insurance that the City purchased for its LEOFF 1 members. The long-term care insurance provides $130 per day for benefits for nursing home care, home care, and assisted living. The daily benefit rate includes a five percent compounded inflation benefit, and the daily benefit rate has risen to approximately $150 per day since the purchase of the insurance policy. The policy includes a five year benefit for any level of care. Mr. Wood recalled that the City had attempted to insure 78 LEOFF I members. A number of members were not insurable, some were unreachable, and some refused to apply. Of those members who were uninsurable under Allianz, five were insured by another insurance provider. At the present time, the City insures 57 LEOFF 1 members. E. Correspondence 1. Letter dated June 12, 2002 from Mary Jane Tinsley regarding Fire Relief and Pension widow's pension. Ms. Tinsley's letter was provided to the Board for information purposes only. The letter will be presented to the Fire Relief and Pension Board at its next meeting for response. F. LEOFF I Annual Medical Insurance Update Each year a medical insurance update letter and questionnaire form is mailed to LEOFF I retired members in order to obtain updated information with regard to members' medical, employment and insurance status. The update form and letter was presented to the Board members for information only. The updates will be mailed to the retirees later this month. V.
