A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF AUBURN,
WASHINGTON, AUTHORIZING THE MAYOR AND CITY CLERK OF
THE CITY OF AUBURN TO EXECUTE AN AGREEMENT FOR LONG-
TERM HEALTH CARE INSURANCE FOR UNAFFILIATED CITY
EMPLOYEES THROUGH UNUMPROVIDENT, UNUM LIFE
INSURANCE COMPANY OF AMERICA.
WHEREAS, the City of Auburn desires to establish a long-term health
care plan for its regular, full time, unaffiliated employees; and
WHEREAS, the City desires to pay the premium for a base amount and
allow employees to individually "buy up" additional coverage; and
WHEREAS, it is in the best interest of the City to establish this medical
benefit; and
WHEREAS, the Unum contract provides the best return on the City's
investment; and
WHEREAS, the Unum contract provides the best "buy up" provisions for
its employees.
NOW, THEREFORE, THE CITY COUNCIL OF THE CITY OF AUBURN,
WASHINGTON, HEREBY RESOLVES AS FOLLOWS:
Section 1. Purpose. The Mayor and City Clerk of the City of Auburn,
Washington are hereby authorized to execute an agreement between the City
of Auburn and UnumProvident, effective December 1, 2002. A copy of said
agreement is attached as Exhibit "A" and incorporated by reference.

Section 2. The Mayor is hereby authorized to implement such
administrative procedures as may be necessary to carry out the directions of
this legislation.
Section 3. The City retains the right to modify or repeal the long-term
care plan at its discretion, without prior notification to City employees.
Section 4. This resolution shall be effective December 1, 2002 in full
force and effect upon passage and signatures hereon.
