MINUTES
I. CALL TO ORDER
Chair Pro Tem Bud Larson called the meeting to order at 10:00 a.m. in the
Council Conference Room located at Auburn City Hall, 25 West Main Street in
Auburn. Chair Pro Tem Larson and the following Board members were present:
Member Bill Peloza, Member Roger Thordarson, and Member Russ Vandver.
Chair Jim Kelly was excused. Also present were Board Attorney Joseph Beck
and Board Secretary Danielle Daskam.
II. APPROVAL OF MINUTES
A. Minutes of the Auoust 2. 2005 LEOFF Board Meetinq
Motion by Member Thordarson, second by Member Peloza to approve the
August 2, 2005 minutes.
MOTION CARRIED. 4-0.
III. UNFINISHED BUSINESS
A. Pendino Disabilitv Leave/Retirement Applications
1. John T. Dodd
Effective Date of Disability: July 22, 2005
Last Day of Service: July 21,2005
On August 2, 2005, the LEOFF Board approved disability leave for
Mr. Dodd effective July 22, 2005 with last day of service on July 21,
2005. Mr. Dodd submitted his monthly physician's report, which is
included in the agenda packet.
Motion by Member Peloza, second by Member Thordarson to
accept the monthly physician's report and continue Mr. Dodd's
disability for an additional month or until cessation of disability,
whichever occurs first.
MOTION CARRIED UNANIMOUSLY. 4-0.
B. Medical/HearinoNision Claims

Claim for payment in the amount of $117.54 for medical services
received on October 18, 2004.
Mr. Fisher's claim was tabled on June 7, July 5, and August 2,
2005. No new information was received.
Motion by Member Peloza, second by Member Thordarson to deny
the claim pursuant to Board Policy 9.01 (i).
MOTION CARRIED. 4-0.
2. Don Casselman
Claim for reimbursement of vision expenses in the amount of
$329.00.
Mr. Casselman's claim was tabled on August 2, 2005 pending
further clarification of Mr. Casselman's vision worksheet. No new
information was received.
Motion by Member Thordarson, second by Member Peloza to table
Mr. Casselman's until the October Board meeting.
MOTION CARRIED. 4-0.
IV. NEW BUSINESS
A. Disability Leave/Retirement Applications
No new application for disability leave/retirement was received.
B. Appeals/Reouests for Reconsideration
No appeal or request for reconsideration was filed.
C. Medical/HearinoNision Claims
1. Cali Ann Edwards
Claim for reimbursement of lenses and frames expenses in the
amount of $226.00. (Ms. Edwards has not received vision reimbursement
within the last year.)
Motion by Member Peloza, second by Member Vandver to approve
the claim in the amount of $200 for lenses and frames, pursuant to
LEOFF Board policy.
MOTION CARRIED. 4-0.

2. Bruce Caldwell
Claim for payment for hearing aid repairs in the amount of $270.00.
LEOFF Board members previously reviewed and denied this claim
in June 2005 finding that the request for reimbursement was for
routine maintenance.
Motion by Member Peloza, second by Member Vandver to deny the
claim.
The Board further reviewed the claim and noted that Mr. Caldwell
resubmitted the request with additional information showing that
the claim is for repairs and not maintenance, as Costco does not
charge for normal maintenance.
Member Peloza withdrew his motion to deny the claim.
Motion by Member Thordarson, second by Member Vandver to
approve the claim in the amount of $270.00.
MOTION CARRIED UNANIMOUSLY. 4-0.
3. Michael Corkins
Request for reimbursement of medical expenses incurred on May
28, 2005 in the amount of $22.1 O.
Board members noted that the claim was not accompanied by a
Regence explanation of benefits statement, and on August 30,
2005, the Board Secretary sent a letter to Mr. Corkins requesting
the insurance explanation of benefits statement.
Motion by Member Vandver, second by Member Thordarson to
table the claim until the next regular Board meeting.
MOTION CARRIED. 4-0.
D. Correspondence
1. Letter dated July 20, 2005 from J. E. Fischnaller regarding
Snohomish County District No. 1 v. Snohomish County Disability
Board.

Chair Pro Tem Larson acknowledged receipt of Mr. Fischnaller's
correspondence. Chair Pro Tem Larson suggested that any
discussion of the letter be postponed until the October Board
meeting.
E. Discussion
1. New Medicare Part D Voluntary Prescription DruQ Coveraoe
This item is pending documentation from the Association of
Washington Cities. No new information was received.
