MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Council
Conference Room located at Auburn City Hall, 25 West Main Street in Auburn.
Chair Kelly and the following Board members were present: Chair Pro Tem Bud
Larson (member-at-Iarge), Member Bill Peloza, Member Roger Thordarson, and
Member Russ Vandver. Also present were Board Attorney Joseph Beck and
Board Secretary Danielle Daskam.
II. APPROVAL OF MINUTES
A. Minutes of the Julv 5. 2005 LEOFF Board Meetinq
Motion by Chair Pro Tem Larson, second by Member Thordarson to
approve the July 5, 2005 minutes.
MOTION CARRIED UNANIMOUSLY. 5-0.
III. UNFINISHED BUSINESS
A. Pendinq Disabilitv Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Medical/HearinqNision Claims
1. Fred Fisher
Claim for payment in the amount of $117.54 for medical services
received on October 18, 2004.
Mr. Fisher's claim was tabled on June 7 and July 5, 2005, pending
the receipt of the Regence Explanation of Benefits Statement. No
new information was received.
Motion by Chair Pro Tem Larson, second by Member Thordarson
to table Mr. Fisher's claim for an additional month.
MOTION CARRIED UNANIMOUSLY. 5-0.
2. Don Casselman
Claim for reimbursement of vision expenses in the amount of
$329.00.

Mr. Casselman's claim was tabled on June 7, 2005 pending the
receipt of the Regence Explanation of Benefits Statement for his
vision exam. Mr. Casselman submitted the requested information
and it was included in the Board's packet.
Board members reviewed Mr. Casselman's vision worksheet, the
billing from the optometrist and the Regence explanation of
benefits statement. Board members concluded that the vision
worksheet completed by the claimant was not consistent with the
documentation.
Motion by Chair Pro Tem Larson, second by Member Thordarson
to table the claim and return the claim for further clarification.
MOTION CARRIED UNANIMOUSLY. 5-0.
IV. NEW BUSINESS
A. Disability Leave/Retirement Applications
1. John T. Dodd
Effective Date of Disability: July 22, 2005
Last Day of Seryice: July 21, 2005
On July 18, 2005, LEOFF Board Chair Jim Kelly tentatively
approved disability leave for John Dodd effective July 22, 2005,
subject to full Board review and approval. Mr. Dodd's application
for disability leave/retirement includes physicians' statements from
Dr. Anna T. Curtis dated July 18, 2005 and Dr. Mike Moore dated
July 18, 2005.
Motion by Chair Pro Tem Larson, second by Member Vandver to
confirm the Chair's action and approve disability leave for Mr. Dodd
effective July 22, 2005 with last day of service on July 21, 2005.
MOTION CARRIED UNANIMOUSLY. 5-0.
B. Appeals/Requests for Reconsideration
No appeal or request for reconsideration was filed.

Claim for reimbursement of lenses and frames expenses in the
amount of $220.00. (Ms. Darough has not received vision
reimbursement within the last year.)
Motion by Member Peloza, second by Chair Pro Tem Larson to
approve the claim in the amount of $200.00 per Section 9.02
relating to claims for corrective lenses and frames.
MOTION CARRIED UNANIMOUSLY. 5-0.
D. Correspondence
1. Letter dated July 4,2005 from Ed Henderson regarding claim.
Chair Kelly acknowledged receipt of Mr. Henderson's
correspondence.
E. Discussion
1. Policy and Procedure Amendment - Medical Claims Procedure
Board members to consider the following proposed amendment to
Section 9.01 (I) of the LEOFF Policy and Procedure Manual:
I. All claims, except prescription claims, shall be
submitted to the Board within ninety (90) days of date
processed/paid by the insurance documentation. The
Board will review the submitted claim at the next full
Board meetinq. If all documentation is submitted
properly the Board will consider reimbursement of
expenses. However. if for any reason. the claim for
payment/reimbursement is insufficient. at the
discretion of the Board. it may table the claim until the
next meetinq or until receipt of the requested
documentation. The claim may be tabled for three
consecutive Board meetinqs. If, after three (3) Board
meetinQs the Claimant has not provided the Board with
the requested documentation. the claim for
payment/reimbursement may bc denied pendinq
receipt ef the roquestod information. F::liluro to comply

Motion by Member Peloza, second by Chair Pro Tem Larson to
suspend Policy (rule) 10.01 and 10.02 in order to review the
proposed amendment to Policy 9.01 (I).

MOTION CARRIED UNANIMOUSLY. 5-0.
Motion by Member Peloza, second by Chair Pro Tem Larson to
adopt the amendment to Policy 9.01 (I) as presented on the
agenda.
Member Peloza spoke in favor of the motion noting that the
proposed amendment formalizes Board practice.
Board Attorney Beck suggested that the last sentence of the
proposed amendment be changed to read in part as follows: "....
the claim for payment/reimbursement may be rejected." Board
members concurred and Member Peloza withdrew the previous
motion.
Motion by Member Peloza, second by Chair Pro Tem Larson to
adopt the amendment to Policy 9.01 (I) as revised to read as
follows:

2. New Medicare Part D Voluntary Prescription Druo Coveraoe
Chair Kelly advised that the presentation on the new Medicare Part
D Voluntary Prescription Drug Coverage will be held over to the

next meeting in anticipation of information from the Association of
Washington Cities.
3. Continued Discussion from June 7. 2005 Meetino Reqardino
Annual Eye Examinations
Board Attorney Beck reported that his research revealed there is
no provision in the Revised Code of Washington or Washington
Administrative Code that would limit one eye examination per year.
Policy 9.01 (K) provides that medical expenses which are
necessary, shall be reimbursed. Policy 9.02(C) provides for
reimbursement of eye examination, but no time limit is provided.
The policies also provide that lenses and frames (up to $200.00)
will be reimbursed annually. There is no limit on the frequency for
eye examinations if the examination is medically necessary.
V. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:34 a.m.
