MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Council
Conference Room located at Auburn City Hall, 25 West Main Street in Auburn.
Chair Kelly and the following Board members were present: Chair Pro Tem Bud
Larson (member-at-Iarge), Member Bill Peloza, Member Roger Thordarson, and
Member Russ Vandver. Also present was Board Secretary Danielle Daskam.
II. APPROVAL OF MINUTES
A. Minutes of the September 6. 2005 LEOFF Board MeetinQ
Motion by Member Peloza, second by Member Thordarson to approve the
September 6, 2005 minutes.
MOTION CARRIED UNANIMOUSLY. 5-0.
III. UNFINISHED BUSINESS
A. Pendinq Disabilitv Leave/Retirement Applications
1. John T. Dodd
Effective Date of Disability: July 22, 2005
Last Day of Service: July 21, 2005
On August 2, 2005, the LEOFF Board approved disability leave for
Mr. Dodd effective July 22, 2005 with last day of service on July 21,
2005. Mr. Dodd submitted his monthly physician's report.
Motion by Member Peloza, second by Chair Pro Tem Larson to
accept the monthly physician's report and continue Mr. Dodd's
disability leave for an additional month or until cessation of the
disability, whichever occurs first.
MOTION CARRIED UNANIMOUSLY. 5-0.
B. Medical/HearinqNision Claims
1 . Don Casselman
Claim for reimbursement of vision expenses in the amount of
$329.00 $205.39.

Mr. Casselman's claim was tabled on August 2 and September 6,
2005 pending further clarification of Mr. Casselman's vision
worksheet. Mr. Casselman submitted a revised vision worksheet
for the Board's consideration.
Board members reviewed Mr. Casselman's new information.
Committee members questioned whether Mr. Casselman's vision
provider accepted Regence's reimbursement as payment in full,
and whether Mr. Casselman should have paid the difference
between the amount billed by the provider and the allowed amount
paid by Regence.
Motion by Member Peloza, second by Member Vandver to table
Mr. Casselman's claim in the amount of $205.39 until such time as
the Secretary can confirm with the provider the balance of the
vision exam.
MOTION CARRIED UNANIMOUSLY. 5-0.
2. Michael Corkins
Request for reimbursement of medical expenses incurred on May
28, 2005 in the amount of $22.10.
Mr. Corkins submitted a Regence explanation of benefits
statement. The Committee members noted that the amount
requested on the claim form ($22.10) conflicts with the amount
appearing on the Regence explanation of benefits statement
($32.84).
Motion by Member Peloza, second by Chair Pro Tem Larson to
table the claim pending receipt of additional information from Mr.
Corkins.
MOTION CARRIED UNANIMOUSLY. 5-0.
IV. NEW BUSINESS
A. Disabilitv Leave/Retirement Applications
No new application for disability leave/retirement was received.
B. Appeals/Requests for Reconsideration
No appeal or request for reconsideration was filed.

1. Robert S. Phillips
Claim for reimbursement of medical expenses in the amount of
$252.07, Medicare claim number 05201838121000, Regence
explanation of benefits claim statements 0520695017800 and
0524195018300.
Motion by Chair Pro Tem Larson, second by Member Thordarson
to approve Mr. Phillips' claim in the amount of $252.07.
MOTION CARRIED UNANIMOUSLY. 5-0.
D. Correspondence
1. Letter dated July 20, 2005 from J. E. Fischnaller regarding
Snohomish County District No. 1 v. Snohomish County Disability
Board.
Chair Kelly acknowledged receipt of Mr. Fischnaller's letter. Chair
Kelly noted that Mr. Fischnaller's letter and the court case,
Snohomish County Fire District No. 1 v. Snohomish County
Disability Board underscores the discretion held by LEOFF Boards
in providing LEOFF 1 benefits.
E. Discussion
1. Dental Insurance
Member Vandver suggested looking into the cost of providing
dental insurance for LEOFF 1 retirees. Chair Kelly indicated he
would work with the Human Resources Department to provide the
requested information relating to dental insurance for LEOFF 1
retirees.
2. Lonq-term Health Care
Chair Pro Tem Larson requested that the Board receive an update
from the Human Resources Department on the City-purchased
long-term care insurance for LEOFF 1 members. Chair Kelly will
request the Human Resources Director provide an update on the
LEOFF 1 long-term care insurance program.
3. Suqqestion for Annual Report
Member Peloza suggested that the Board provide the Auburn City
Council and Mayor Lewis with a brief annual report of the Board's

activities. Chair Kelly offered to confer with the Board Attorney on
the suggestion.
4. New Medicare Part D Voluntary Prescription Druq CoveraQe
Chair Kelly reported that City is waiting for documentation from the
Association of Washington Cities regarding Medicare Part D. The
Association of Washington Cities is acting on behalf of the City in
notifying eligible LEOFF 1 retirees about the new Medicare Part D
program, but the City has not received a copy of the notification
that has been sent to the retirees.
V. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:24 a.m.
