CALL TO ORDER Chair Kelly called the meeting to order at 10:00 AM at Auburn City Hall, 25 West Main Street, Auburn, WA. Chair Jim Kelly and the following Board members were present: Chair Pro Tern Bud Larson, and Member Russ Vandver. Member Stacey Brothers arrived at 10:05 AM. Member Gene Cedno was excused. Also present were Ed Henderson, Associate City Attorney Karen Gulliver, and Board Secretary Danielle Daskam. II. APPROVAL OF MINUTES A. Minutes of the October 7, 2002 LEOFF Board Meeting* Motion by Member Vandver, second by Chair Pro Tem Larson to approve the October 7, 2002 minutes as distributed. MOTION CARRIED UNANIMOUSLY. 3-0. III. UNFINISHED BUSINESS A. Pending Disability Leave/Retirement Applications 1. Marion F. Dukes Effective Date of Disability: September 21, 2002 Last Day of Service: September 20, 2002 On October 7, 2002, the LEOFF Board approved disability leave for Marion Dukes effective September 21, 2002. In addition, the Board waived the requirement for monthly physician reports during Mr. Dukes' disability leave. Motion by Chair Pro Tern Larson, second by Member Vandver to continue Mr. Dukes' disability leave for an additional month or until cessation of disability, whichever occurs first. MOTION CARRIED UNANIMOUSLY. 3-0. Ray Nixon On August 5, September 3, and October 7, 2002, the LEOFF Board tabled Mr. Nixon's claim for payment pending receipt of Regence and Medicare explanation of benefits statements. No new information was received. a. Request for reimbursement for medical services received on March 13, 2002 in the amount of $69.00. (No Medicare or Regence Explanation of Benefits submitted.) Motion by Chair Pro Tem Larson, second by Member Vandver to deny the claim pending receipt of the requested information. MOTION CARRIED UNANIMOUSLY. 3-0. 2. Raymond O.qden On August 5, September 3, and October 7, 2002, the LEOFF Board tabled Mr. Ogden's claim for payment pending receipt of Regence and Medicare explanation of benefits statements. No new information was received. a. Request for reimbursement of medical expenses received on June 13, 2002 in the amount of $182.00. (No Medicare or Regence Explanation of Benefits submitted.) Motion by Member Vandver, second by Chair Pro Tern Larson to deny the claim pending receipt of the requested information. MOTION CARRIED UNANIMOUSLY. 3-0. 3. Ed Henderson Request for reimbursement of pre-authorized dental expenses in the amount of $795.00. (Member Brothers arrived at this time) Chair Kelly noted that Mr. Henderson's attendance at the meeting. Chair Pro Tern Larson recalled that at the Board's October 7, 2002 meeting the Board Pre-authorized the medical/dental expense. Motion by Chair Pro Larson, second by Member Vandver to approve reimbursement in the amount of $795.00. MOTION CARRIED UNANIMOUSLY. 4-0. IV. NEW BUSINESS A. Disability Leave/Retirement Application.~ No new application for disability leave/retirement was submitted. B. Appeals No appeal was filed. C. Medical Claims 1. Coy Franklin Grail a. Regence Explanation of Benefits Claim No. 0224933055700 processed on September 27, 2002 for medical services received on August 3 and 15, 2002 in the amount of $134.20. Board members reviewed the claim for payment documents. Motion by Member Vandver, second by Chair Pro Tem Larson to approve Mr. Grail's claim in the amount of $134.20. MOTION CARRIED UNANIMOUSLY. 4-0. D. Fire Department LEOFF Board Representative Election Nomination forms were mailed and three nominations were received by the deadline on October 29, 2002 at 5:00 PM. Election ballots will be mailed November 8, 2002. Nominations were received from: Mick Crawford, Rudolph Peden, and Russ Vandver. Letter dated October 11, 2002 from Philip A. Talmadge regarding time limits for submission of LEOFF 1 claims for reimbursement of medical expenses. Chair Kelly acknowledged the receipt of Mr. Talmadge's letter written on behalf of Mr. Ed Henderson and Retired Fire Fighters of Washington. Chair Kelly referred Mr. Talmadge's letter to the Board's attorney for response. V. ADJOURNMENT There being no further business to come before the Board, the meeting adjourned at 10:13 AM.
