MINUTES
I. . CALL TO ORDER
Chair Jim Kelly called the rr►eeting to order at 10:00 a.m. in the Council.
Conferenoe Room located at Auburn City Hall, 25 1Nest Main Street in Aubum.
Cha'ir Kelly and the following Board members were present: Chair Pro Tem Bud
Larson, Member �rginia Haugen, and Member Bill Petersen. Member Lynn
Norman was excused. Also present were Board Attomey Steve Gross and
Board Secretary Danielle Daskam.
II. AGENDA MODIFICATIONS
A claim from RichaFd Schneide�was added to the agenda.
III.. APPROVAL OF MINUTES
A. Minutes of ihe Auaust2,2011 LEOFF Board Meetinq
Motion by Chair Pro Tem Larson second 6y Member Petersen to approve
the August 2, 2011 minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 4-0.
IV. UNFINISHED BUSINESS
A. Pendin4 Disabilitv Leave/Retirement Aa�licafions
There was no pending disability leave application for reView.
B. Pendinq Medical/Hearinq�sion Dental Claims
1. Sam Mulkev
Request for reimbursement of long term, rehabilitation and nursing
care costs in the amount of$1;364.38:
On June 7, July 5, and August. 2, 2011, the Board tabled Mr.
� Mulkey's claim pending the receipt of additional information in the
form of a more current billing statement from La Mesa
Rehabilitation reflecting Mr. Mulkey's payment. No new information
was received.
Motion by Chair Pro Tem Larson, second by Member Petersen to
deny the claim pending receipt of tfie requested information.
MOTION CARRIED UNANIMOUSLY. 4-0.

Request for long term care costs for the month of July, 2011 in the
amounYof$2,132.80.
Motion by Chair Pro Tem Larson, second by MemberPetersen to
approve Ms.'Darough's claim in the amount of$2,132.80.
MOTION CARRIED UNANIMOUSLY. 40.
3. Roaer H. Lee
Request for reim6ursement of dentaf expenses in the amount of
$184.30 for services received on AugusY 22, 2011. Mr. Lee has
not received dental expense reimbursement in 2011.
Motion by Member Petersen, second by Chair'Pro Tem Larson to
approve Mr. Lee's claim in the amount of$184.30.
MOTION CARRIED UNANIMOUSLY. 4-0.

5. John Calkins
Request for reimbursement of dental expenses in the amount of
$738.00 for services received on June 21, 2011. The Board has
previously found 4hat Mr. Calkins' dental services are medically
necessary.
Motion by Member Petersen, second by Chair Pro Tem Larson to
approve Mr. Calkins' claim in the amount of$738.00.
MOTION CARRIED UNANIMOUSLY. 4-0.
6. Richard Schneider
Request for reimtiursement of dental expenses in the amount of
$247.00 for services received on August 31, 2011. In 2011, Mr.
Schneiderhas expended the maximum amount allowed by policy.
