The Contractor is hereby directed to make the changes to the Contract as described herein. This document and all attachments arc
a supplement to the contract. All work,materials and measurements for the type of construction involved shall be in accordance
with the contract documents of the above maned project unless stated otherwise in this change order.
Summary of Proposed Changes:
1 The Contractor shall install a fire hydrant along Nathan Ave SE between 57th Ct SE and Evergreen
Way SE. Price includes all labor,equipment,and materials to install a complete fire hydrant assembly,
including but not limited to mobilization,traffic control labor,wet tap of existing 12 inch diameter
main,and installation of the ftre'hydrant assembly, including 6 inch diameter ductile iron pipe,
temporary pavement patch,saw cutting,concrete sidewalk replacement,asphalt trench patch,and
landscape restoration. Work shall conform to Section 7-14 of the Special Provisions. 2 working days
are also added to the Contract.
The Contract time is extended by 2 days.

I Added Contract days only due to delays in the owner supplyed SCADA Control Panel and delays in
critical material deliveries from suppliers which has affected the construction critical path.
The Contract time is extended by 55 days.
