The Contractor is hereby directed to make the changes to the Contract as described herein This document and all attachments are a
supplement to the contract All work, materials and measurements for the type of construction involved shall be in accordance with
the contract documents of the above named project unless stated otherwise in this change order
Summary of Proposed Changes:
1 During the construction of the parking lot improvement, unsuitable subgrade was found in a few
areas Over excavation is needed and filled with ballast to a suitable subgrade Dirt shoulder
was ripped up during the boardwalk and parking lot construction, gravel shoulder is needed to
stabilize the shoulder and reduce mud tracking
2. During the construction of the shoulder improvement, unsuitable subgrade was found in a few
areas Over excavation is needed and additional gravel filled to a suitable subgrade Additional
3/8" gravel needed to match grade between the boardwalk's headwall and the compacted gravel
ramp
3 After a week of the compacted gravel pedestrian trail ramp being in place Due to heavy rain
and foot traffic the compacted gravel does not seem to hold up Gravel begins to come loose
and does not meet ADA guidelines Pervious concrete was replaced for a more stable /harden
surface
4 The concrete strip that connects the boardwalk to the existing asphalt sidewalk on Main St does
not match grade in some areas and could be a tripping hazard Repair needed to provide the
smooth transition
