The Contractor is hereby directed to make the changes to the Contract as described herein This document and all attachments are a
supplement to the contract All work,materials,measurements and unit bid prices for the type of construcnon involved shall be in accordance
with the contract documents of the above named project unless stated otherwise in this change order.
Summary of Proposed Changes:
1 Replace damaged 8-inch clay sewer pipe on sewer easement between G St NW and H St NW just north of West
Main St(approximatly 30 feet of pipe going south from manhole 708-85). New pipe shall be 8-inch SDR35
Replace cleanouts and side sewer tees and pipe as required. Restore surface as directed by the Engineer.
Contractor has been give verbal authorization to start the work on 6/28/12.
2. Replace damaged side sewer tee at 1301 67th St SE Unit 19 Restore pavement after installation. Contractor has
been given verbal authorization to start the work on 7/16/12.
The Contract time is extended by 0 days.
