MINUTES
I: CALL TO ORDER
Chair Jim Kelly called the meetingto order at 10:00 a.m. in the Council
Conference Room located at Aubum City Hall, 25 West Main Street in Auburn.
Chair Kelly and the following Board members were present: Bill Petersen, John
Holman, Wayne Osbome, and Kurt Vogel. Also present were Board Attomey
Steve Gross and Board Secretary DanielleDaskam..

III. APPROVAL OF MINUTES
A. Minutes of the Mav 1. 2012 LEOFF Board Meetina`
Motion by Chair Pro Tem Petersen; second by Member Osborne to
approve the May1, 2012 minutes as distributed.
MOTION CARRIED UNANIMOUSLY: 5-0
N. UNFINISHED BUSINESS
A. Fendinq Disabilitv Leave/RetiremeM Aoalications
Tljere was no pending disabiliry leave/retirement application.
B. Pendina MedicaVHearinaNision Dental Claims
, 1. Ed Henderson
Request for reimbursement of inedical expenses in the amount of
$2,057.00 for ongoing treatment.
On February 7, March 6 and.April 3, 2012, the Board tabled Mr.
Henderson's claim pend'ing the reoeipf of a claim for payment form
and explanation of benefits statements from Medicare and
insurance. On May 18�, the Board denied the claim. Since then,
Mr. Henderson has submitted the enclosed information as an
update to his rejected claim.
The Board acknowledged receipt of information from Mr.
Henderson advising 4hat he was wai#ing for additional information
from Medicare and Regence. There was no action required by the
Board.

1. Rudv Peden
a. Request for reimbursement of vision expenses in the
amount of $99.00 for services received on October 31,
2011.
Motion by Chair Pro Tem Petersen, second by Member
Vogel to approve the claim in the amount of$99.00
Board members agreed that. the Board Secretary should
transmit a letter to Mr. Peden reminding him of the need to
promptly submit claims. LEOFF Policy9.01(I) requires that
all claims, except prescription and Medicare Part B claims,
shall be submitted to the Board within ninety days.
MOTION CARRIED UNANIMOUSLY. 5-0 •
b. Request for reimbursement of $180.00 in medical expenses
for services received February 12; 2012 —April 20, 2012.
Motion by Chair Pro Tem Petersen, second by Member
Osbome to table Mr: Peden's claim pending the receipt of
the insurance explanation of benefits statements for the
services received.
MOTION CARRIED UNANIMOUSLY. 5-0.
2. Bill Pitzl
Request for reimbursement of dental expenses in the amount of
$184.50 for services received on May 9, 2012.

3. James Fisk
Request for reimbursement of vision expenses in the amount of
$272.00 for services received on April 26, 2012. Mr. Fisk has not
received vision expense reimbursement this year.
Motion by Chair Pro Tem Petersen, seco�d by Member Osbome to
approve the claim to the policy limit of$270.
MOTION CARRIED UNANIMOUSLY. 5-0
4. Ed Henderson
RequesY for reimbursement of derrtal expenses in the amount of
$90.25 for services received on May 15, 2012. Mr. Henderson's
dental claims have been previously found to be medically
necessary.

5. Sammv Mulkev
Request for reimbursement of dental expenses in the amount of
$154.00 for services received on April 25, 2012. Mr. Mulkey has
not received den4al reimbursement in 2012.
Motion by Chair Pro Tem Petersen, second by Member Vogel to
approvethe daim in the amount of$154.00.

6. Ro9er Lee
Reguest for reimbursement of dental expenses in the amount of
$319.20 for services receiVed on Aprit 30, 2012. Mr. Lee's dentaf
claims have been previously found to be medically necessary. He
has not received dental reimbursement in 2012.
Motion by Chair Pro Tem Petersen, second by Member Osborne to
approve the claim in the amountof$319.20.
MOTION CARRIED UNANIMOUSLY. 5-0
7. Murrav Board
Reguest for reimbursement of dental expenses in the amount of
$179.00 for services received on May 2, 2012. Mr. Board has not
received dental reimbursement in2012.
Motion by Ghair Pro Tem Petersen;second by.Member Osborne to
approve the claim in tHe amount of$179.00
MOTION CARRIED UNANIMOUSLY. 5-0
8. Doris Darouah
Request for reimbursement of long term care costs for the month of
April 2012 in the amourrt of$4,830.44.
Motion by Member Vogel, second by Chair Pro Tem Petersen to
approve the claim in the amount of$�4,830.44.
There was discussion among the Board members relating to fhe
process for long term care and reimbursement: of expenses and
whether there was follow-up by the Board on the member's existing
condition and medical necessity for continuing care.
MOTION CARRIED UNANIMOUSLY. 5-0
9. Stan Laatsch
Mr. Laatsch's claim was added as an agenda modification.
Request for reimbursement of dental expenses in fhe amount of
$265.00 for servicea received on May 23; 2012. Mr. Laafsch has
not received dental reimbursement in 2012.

Motion by Chair Pro Tem Petersen, second by Member Osbome to
approve the claim in the amount of$265.00.

D. LEOFF Board Policies and Procedures
Chair Kelly reminded Board members of the pending review of the LEOFF
Board Policies and Procedures. He en"couraged all members to review
the policies and be prepared to propose any amendments at ttie July 3,
- 2012 meeting.
The Board Secretary will conduct a survey of dental and vision expenses.
Board Attomey Gross will perform a legal review of the current policies
and procedures.
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjoumed at 10:30 a.m.
