MINUTES
I. CALL TO ORDER

ChairJim Kelly called the meeting to orderat 10:00 a.m. in the Counal
Conference Room located at Auburn City Hali, 25 West Main Street in Aubum.
ChairKelly and the following Board members were present: Bill Petersen, John
Holman, and Wayne Osborne. Member Kurt Vogel was excused. Also present
were Scot Pondelick, Board Attorney Steve Gros§, Records Clerk Stephanie
Fuller and City Clerk Danielle Daskam.

1. Bruce Caldwell
On November 6, 2012, December 4, 2012, and January 8, 2013,
the Board tabled the claim describetl below. Mr. Caldwell has
submitted the enclosed documentation in support of his claim.
a. Request for reimbursement ofi medical equipment in the
amount of $159.98.
The Board reviewed the additional information submitted by Mr.
Caldwell in support of his request, including a statement from his
physician.

RequesY for reimbursement of dental expenses in the amount of
$843.00 for seroices received on January 8, 2013. Mr. Husebyhas
not received dental reimbursement in 2013.
Motion by Member Osbome, second by Chair Pro Tem Petersen to
approve the claim in the amount of $843.00.
MOTION CARRIED UNANMOUSLY. 4-0
2. Robert Almv
Request for reimbursement of dental expenses in the amount of
$109.00 forservices received on January 15, 2013. Mr. Almy has
not received dental reimbursement in 2013.
Motion by Chair Pro Tem Petersen, second by Member Osbome to
approve the claim in fhe amount of $108.00.

3. Don Webb
Request for reimbursement of dental expenses in the amourrt of
$125.00 for services received on January 4, 2013. Mr. Webb has
not received dental reimbursement in 2013.

approve the claim in the amount of $125.00..

Motion by Chair Pro Tem Petersen, second by Member Osbome to
approve the claim in the amount of $20.00.

Board members requested the Board Secretary obtain a copy of
the Valley Regional Fire Authority retirees' health care coverage
plan.
5. Loren Anderson
Request for reimbursement of dental expenses in fhe amount oi
$433.20 for services receivetl on January 9, 2013. Mr. Anderson
has not received dental expense reimbursement in 2013.
Motion 6y Member Holman, second by Member Osbome to
approve the claim in the amount of $433.20.
MOTION CARRIED UNANIMOUSLY. 4-0
6. Fred Fisher
Request for reimbursement of dental expenses in the amounf of
$90.00 for services received on December 12, 2012. Mc fisher
did not receive reimbursemeM for dental expenses in 2012.
Motion by Chair Prb Tem Petersen, second by Member Osbome to
approve the claim in the amount of $90.00.
MOTION CARRIED UNANIMOUSLY. 4-0
7. Russ Vandver
Request for reimbursement of inedical expenses in the amaunt of
$360.00 for services received on September 4 and October 16,
2012 and processed by insurance on November 7, 2012.
Motion by Chair Pro Tem Petersen, second by Member Osbome to
approve the claim in the amount of $360.00.

a. Request for reimbursement of long term care costs for the
month of December, 2012 in 4he amount of $11,031.19.
b. Request for reimbursement of long term care costs for the
month of January 2013 in the amount of $11,036.00.
Motion by Member Holman, second by Chair Pro Tem Petersen to
approve the claims for long term care for December 2012 and
January 2013 in the amounts of $11,031.19 and $11,036.00,
respectively.
MOTION CARRIED UNANIMOUSLY. 4-0
