This agreement is entered into between BRAVO CARPET CARE INC and
AUBURN SENIOR CENTER for the agreed amount of$l,716 annually plus sales tax where
applicable.
Bravo Carpet Care Inc. will clean and maintain the carpet at:
Auburn Senior Center
808 9th Street SE
Auburn WA 98002-6233
Per Frequency Schedule
Terms
This program will be billed per the scheduled cleaning. Terms are net 30 days. late payment
charge of 1 Y:z% will apply.
Cancellation
A thirty (30) day written notice of cancellation is required. All outstanding invoices will be due
upon cancellation.
Insurance
Bravo Carpet Care Inc. agrees to carry insurance on all work performed.
