Dater/; ")/ cx‘3 Page 1 of 2 Exhibit A Scope of Services City of Auburn Agreement AG-C-356-T006 Phase 1 Hillside Evaluation The Consultant shall complete the following: • Complete visual site reconnaissance to assess the current condition of the hillside. • Provide local and global slope stability analysis of the slide area and surrounding hillside in the vicinity of the slide. • Provide recommendations for stabilizing the slope in question, including budget level cost estimates. Phase 1 Deliverables: • Technical Memorandum (draft and final version that incorporates and addresses City comments) that summarizes the hillside evaluation. Phase 2: Design Support The Consultant shall complete the following: • Provide design support for laying the pipe on the side slope including, but not limited to, evaluation of slope stability along the proposed alignment for the replacement storm line, details for the pipe support anchors and spacing of each support along the hillside, special connections to each drainage structure, and specialty details needed at each trail crossing. • Provide additional guidance as necessary during the design and construction of the pipe and slope stabilization measures, if applicable. Phase 2 Deliverables: • Technical Memorandum (draft and final version that incorporates and addresses City comments) that summarizes that the anchor support and other pipe design considerations, pipe and structure anchor details, technical specifications (if needed) approximate unit costs, and any recommendations to address slope stability concerns along the storm lines proposed alignment. AG-C-356 BETWEEN THE CITY OF AUBURN AND TERRACON CONSULTANTS, INC RELATING TO ON-CALL GEOTECHNICAL ENGINEERING SERVICES 1 / THIS AMENDMENT is made and entered into this (ice day of ALLec<P-G , 2013, by and between the CITY OF AUBURN, a municipal corporation of the State of Washington (hereinafter referred to as the "CITY"), and Terracon Consultants, Inc (hereinafter referred to as the "CONSULTANT"), as an Amendment to the Agreement between the parties for AG-C-356 executed on the 4th day of February, 2009 and amended by agreements dated the 21st day of December, 2009, the 21St day of June, 2010, the 13th of December, 2010; the 8th day of November, 2011; and the 19th day of December, 2012. The changes to the agreement are described as follows: 1. CONTRACT TERM: The term of the Agreement for Professional Services is extended to January 31, 2014. 2. SCOPE OF WORK: There is no change in the scope of work. 3. COMPENSATION: There is no change to the amount authorized in the original agreement and subsequent amendments. REMAINING TERMS UNCHANGED: That all other provisions of the Agreement between the parties for AG-C-356 executed on the 4th day of February, 2009 shall remain unchanged, and in full force and effect. This Task Assignment has a completion date of January 31, 2014, and a not-to-exceed amount of $9,200.00. You are not authorized to perform work beyond the completion date, or expend funds above and beyond the not-to-exceed amount. This letter serves as your Notice to Proceed. Also enclosed, please find an executed original of the above-referenced Amendment No. 6. This amendment is for a time extension only. This amendment extends the term of the agreement to January 31, 2014. For the City's tracking and record keeping purposes, please reference AG-C-356 on all correspondence and related material. Our records indicate that your Insurance, as required in Section 8 of the original agreement, will expire on January 1, 2014. Please mail the required certificate(s) of insurance, within two weeks, to Amanda DeSilver, Contracts Administration Specialist, Auburn Public Works Department, at the address noted at the top of this letterhead.
