WA 98082 The Contractor is hereby directed to make the changes to the Contract as described herein. This document and all attachments are a supplement to the contract. All work, materials,measurements and unit bid prices for the type of construction involved shall be in accordance with the contract documents of the above named project unless stated otherwise in this change order. Summary of Proposed Changes: Per the attached revisions, remove two sections of curb and gutter and install new curb and gutter at lower grades on H St NE from station 20 +00 to 22 +20 and station 23 +52.50 to 24 +82.09. Grind and rebuild portions of the pavement as indicated on the revisions to match into the new curb and gutter grades. Stockpile the existing crushed surfacing base course onsite and reuse. Install one new catch basin at the new low point in the curb and gutter at station 20 +75 and connect it to existing catchbasin at station 20 +50. The Contract time is extended by days. Item Sch. This document and all attachments are a supplement to the contract. All work,materials and measurements for the type of construction involved shall be in accordance with the contract documents of the above named project unless stated otherwise in this change order. Summary of Proposed Changes: Contract time is extended by 22 days due to the following reasons: 1. 3rd party delay while waiting for Puget Sound Energy,Century Link and Comcast to relocate their utility poles on H St NE. 2. 3rd party delay while waiting for Puget Sounds Energy to relocate the gas service at 1210 23rd St SE. 3. Delay while waiting for the City to determine the revised replacement limits for the sidewalk on H St NE and additional time needed for the removal and installation of the additional sidewalk determined to be needed by the City.
