MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Council
Conference Room located on the firsf floor of Aubum City Hall,25 West Main
Street in Aubum. Members present included Chair Kelly, Chair Pro Tem Bill
Petersen, Member John Holman, Member Wayne Osbome, and Member-at-
large Kurt VogeL Also present were Board Attomey Steve Gross and Board
Seoretary Danielle Daskam.
II. AGENDA MODIFICATIONS
Claims for payment from Murcay Board and Doris Darough were added to the
agenda.
111. APPROVAL OF MINUTES
A. Minutes of the Februarv 4. 2014 LEOFF Board Meetinq
Motion by Chair Pro Tem Petersen, second by Member Vogel to approve
the minutes of the February 4, 2014 meeting as distributed.
MOTION CARRIED UNANIMOUSLY. 5-0
IV. UNFINISHED BUSINESS
A. Pendinq Disabilitv Leave/Retirement Aaolications
There was no pending disatiility leave/retirement application.
B. Pendina MedicaUHearinaNision Dental Glaims
1. Rudv Peden
a. Request for reimbursement of inedical expenses in the amount
of $232.28 for services receiyed on November 14, 2013 and
processed by insurance on November 26, 2013. No addi4ional
information was received.
Motion by Ghair Pro Tem Petersen, second by Member Vogel
to table the claim an additional month.

Request for reimbursement of dental expenses in the amount of
$178.15 for services received on February 4, 2014. Mr. Lindback
has not received dental reimbursement in 2014.
Motion by Chair Pro T.em Petersen, second by Member Vogel to
approve the claim in the amount of$178.15.
MOTION CARRIED UNANIMOUSLY. 5-0
2. Richard Schneider
Request for reimbursement of vision expenses in the amount of
$334.97 for services received on February 13, 2014. Mr.
Schneider has not received vision expense reimbursement in 4he
last year.
Motion by Chair Pro Tem Petersen, second by Member Vogel to
apprbve the claim in the amount of $334.97 for two sets of
glasses.
MOTION CARRIED UNANIMOUSLY. 5-0
3. Robert Wialev
Request for reimbursement of dental expenses in the amourrt of
$160.00 for services received on February 5, 2014. . Mr. Wigley
has not received dental expense reimbursement in 2014.
Motion by Chair Pro Tem Pe4ersen, second by Member Vogel to
approve the claim in the amount of$160.00.

Wolters has not received dental reimbursement in 2014.
Motion by Member Vogel, second by Chair Pro Tem Petersen
to approve the claim in the amourrt of$115.00.

b. Request for reimbursement of inedical expenses in the amount
of$7.44 for services received on March 28, 2013.
Motion by Member Vogel, second by Chair Pro Tem Petersen
to approVe the claim in the amount of$7.44.

5. Murrav Boartl
Request for reimbursement of vision expenses in the amount of
$109.98 for services received on February 27, 2014. Mr. Board
last received vision reimbursement on March 5, 2013 in the amount
of$89.98.
Motion by Chair Pro Tem Petersen, second by Member Holman to
approve the claim in the amount of$109.98.
MOTION CARRIED UNANIMOUSLY. 5-0
6. Doris Darouah
Request for reimbursement of long term/home health care costs in
the amount of$7,560.00 for the mbnth of February, 2014.
Motion by Member Holman, second by Chair Pro Tem Petersen to
approve the claim in the amount of$7,560.00.

D. Other Discussion

The Board acknowledged receipt of the correspondence from Mr:
Crawford regarding eligibility for Social Security and enroliment in
Medicare.
Chair Pro Tem Petersen stated the LEOFF BoaM Policies and
Procedures require members to enroll in Medicare; premiums for
enrollment are eligible for reimbursement. Chair Pro Tem
Petersen recommended the Board correspond with Mr. Crawford
to advise him to contact Social Security Administration and file for
Medicare Part A and Part B coverage. Board Attomey Gross will
draft a letter for the ChaiPs signature and transmittal to Mr.
Crawford.
2. Correspondence received ftom Fire retiree Alan Kosko
The Board acKnowledged receipt of correspondence from retiree.
Alan Kosko regarding medications he is currently prescribed,
which are not covered under the new Regence Medicare
Advantage Program formulary.
3. Annual Review of LEOFF Board Policies and Procedures
Board members reviewed the LEOFF Board Policies and
Procedures and recommended the following changes:
Section 9.02
A. Active LEOFF 1 Members
Vision insurance coverage is provided for active LEOFF 1
members through ' Reqence BlueShield. If
any balance remains after Reqence
BlueShield, or any other vision plan the LEOFF I mem6er is
enrolled in, has paid its benefrt, the membershould forvvard a
copy of the F�cplanation of Benefits statement together with a
completed Corrective Lenses and Frames Worksheet to the
LEOFF Board for consideration. The LEOFF Board will
reimburse up to the amounts described below. The
reimbursement rates apply after all insurance coverages have
been applied. Any balance due after the LEOFF Board
reimbursement is the responsibility of the LEOFF I member.
Section 8.02(D)(1), Board Attomey Gross will draft additional
language regarding definition of acciderrtal injury as it.relates
to dental expenses.

Section 8.U2(F), Board Attomey Gross will draft proposed
language to replace paragraph F regarding a member's
enroliment in Medicare.
Section 8.03 and Section 9.01, Board Attomey Gross will draft
proposed language regarding durable medical eguipmeM that
requires pre-approval and proof of inedical necessity.
Section 9.01(G) amend to read as follows:
Claims which do not have complete documentation s19a11-m�,a �
be tabled until the next LEOFF Board meeting or until the
required documentation is provided so it is crucial to have the
required paperwork. Only those medical services which are
deemed necessary shall be approved, unless the Board finds
the condition which has caused the need for such medical
service was caused or brought on by dissipation or.abuse.
Determinations of dissipation or abuse and the necessity of
such medical services shall be determined by the Board after
considering 4he medical evaluation of the Board's medical
advisor together with any other relevant evidence.
Section 9.02, correct the numbering of paragraph titled "CorrectiVe
Vision Surgery".
Section 9.03, Medical Claims for Hearing Aids, amend to read in
part as follows: �
The City of Auburn LEOFF Board will authorize a payment for
hearing aid(s) up to an amount determined by the LEOFF
Board designated hearing aid provider (RriseFCostco�e#
�-F�9��). . .
Motion by Chair Pro Tem Petersen, second by Member Vogel to
approve the amendments to the LEOFF Board Policies and
Procedures as discussed.
MOTION CARRIED UNANIMOUSLY. 5-0
4. LEOFF (WSLEA) 2014 Annual Conference
Chair Pro Tem Petersen and Member Vogel will be attending the
annual conference in Chelan May 12-15, 2014.
