MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Council
Conference Room located on 4he first floor of Aubum City Hall, 25 West Main
Street in Aubum. Members present included Chair Kelly, Chair Pro Tem Bilf
Petersen, Member John Holman, Member Wayne Osbome, and Member-at-
large Kurt Vogel. Also present were Board Attomey Steve Gross and Board
Secretary Danielle Daskam.
II: AGENDA MODIFICATIONS
Claims#or payment from Theodore Lindback, Coy Franklin Grall and Doris
Darough were added to the agenda. A proposed amendment to LEOFF Board
Policies and Procedures, Section 8.02(D){1) was also transmitted at the:meeting.
111. APPROVAL OF MINUTES
A. Minutes of the March 4. 2014 LEOFF Board Meetinq
Motion by Member Holman, second by Chair Pro Tem Petersen to
approve the minutes of the March 4, 2014 meeting as distributed.
MOTION CARRIED UNANIMOUSLY. 5-0
IV. UNFINISHED BUSINESS
A. PendinQ_Disabilitv Leave/Retirement Aaaliqtions
There was no pending disabil'ity leave/retirement application.
- B. Pendina Medical/Hearina�sion Dental Claims
1. Rudv Peden '
a Request for reimbursement of inedical e�enses in the amount
of $232.28 for services received on November 14, 2013 and
processed by insurance on November 26, 2013. On February
4 and March 4, 2014, the Board tabled Mr. Peden's claim. No �
further informa4ion was received.
Motion by Chair Pro Tem Petersen, second by Member
Holman to table Mr.,Peden's until the next Board meeting.

No application for disability leave/retirement was received.

No �equest for reconsideration was received.
C. Medical/HearinqNision/Dental Claims
1. Sammv Mulkev
Request for reimbursement of dental expenses in the amount of
$932.00 for services received on February 24 and 27, 2014. Mr.
Mulkey has not received dental reimbursement in 2014.
Motion by Member Vogel, second by Chair Pro Tem Petersen to
approve the claim in the amount of$932.00.
MOTION CARRIED UNANIMOUSLY. 5-0
2. Ed Henderso�
a. Request for reimbursement of'dental expenses in the amount
of $10925 for services received on February 27, 2014. Mr.
Henderson's dental expenses have previously been found to
be medically necessary.
Motion by Chair Pro Tem Petersen, second by Member Vogel
to approve the claim in the amount of$109.25.
MOTION CARRIED UNANIMOUSLY. 5-0
b. Request for reimbursement of dental expenses in the amount
of $1,957.00 for services received on February 24, 2014. Mr.
Henderson's dental expenses have previously been found to
be medically necessary.
Motion by Chair Pro Tem Petersen, second by Member
Holman to approve the claim in the amount of$1,957.00.

approve the claim in the amount of$814.95.

4. Cov Franklin Grall
Request for reimbursement of dental expenses. in the amount of
$138.00 for seryices received on March 27, 2014. Mr. Grall has
not received dental expense reimbursement in 2014.
Motion by Member Vogel, second by Chair Pro Tem Petersen to
approve the claim in the amount of$138.00.
MOTION CARRIED UNANIMOUSLY. 5-0
5. Doris Darouah
Request for reimbursement of long term/home health care costs in
the amount of$8,370.00 for the month of March, 2014.
Motion by Chair Pro Tem Petersen, second by Member Osborne to
approve the claim in the amount of$8,370.00.
MOTION CARRIED UNANIMOUSLY. 5-0
D. Other Discussion
1. Board correspondence to Fire retiree Eugene (Mick) Crawford
Chair Kelly noted a letter dated March 27, 2014 f�om Chair Kelly to
Eugene Crawford regarding application for Medicare benefts for
Board members' informafion.

The Board reviewed the amendments discussed at the last Board i
meeting as well as a proposed amendment to Section 8.02(D)(1). '
Chair Kelly inquired regarding Section 8.03(C)and an example of I
a self help device. ,
Upon further discussion, the Board agreed to approve the addi6on
of the last serrtence of Section 9.01(N) to Section 8.03(C), which
states: "The Boa�d may approve self-Help or comfort items if; in
the Board's sole discre4ion, failure to approve these items will
resutt in the claimant going into a nursing home, and the cost of
the items is less than the costof nursing home care."
Motion by Member Holman, second by Chair Pro Tem Petersen to
approve the amendments as presented with the change to Section
8.03(C) described above.
MOTION CARRIED UNANIMOUSLY. 5-0
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjoumed at 10:19 a.m.
