The Contractor is hereby directed to make the changes to the Contract as described herein. ']'his document and all
attachments are a supplement to the contract. All work, materials,measurements and unit bid prices for the type of
construction involved shall be in accordance with the contract documents of the above named project unless stated
otherwise in this change order.
Summary of Proposed Changes:
This change order adds working days and an additional plan sheet (Sheet 12 attached) to
the Contract to account for additional tree and stump removals that were not included in
the original contract and to provide the removal and replacement of the concrete curb
ramp at the SE corner of 3rd Street NE and A Street NE that was not included in the
original contract. All costs associated with the additional work shall be paid at original unit
bid prices for the applicable items.
The Contract time is extended by
5 days.

Contract Completion and Acceptance
Dear Mr. Chapman;
All of your obligations in regard to this project have been met in accordance with Section 1 -08.5
of the Standard Specifications. The one -year warranty period begins as of the date of this
Contract Completion and Acceptance Letter. The total contract amount for this project, as
shown on your Final Pay Estimate is $161,441.15.
Your retainage in the amount of $8,072.06 will be released approximately 60 days after this
date, provided all necessary releases have been received, and there are no outstanding claims
against the retainage. All approved Labor & Industries Affidavits of Wages Paid from your firm
and all subcontractors on the project have been received.
