MINUTES
I. CALL TO ORDER
Chair Pro Tem Bill Petersen called the meeting to order at 10:00 a.m. in the
Council Conference Room 2 located on fhe second floor of Aubum City Hall, 25
West Main Street in Auburn. Members present included Chair Pro Tem
Petersen, Member John Holman, Member Wayne Osborne, and Member-at-
large Kurt Vogel. Chair Jim Kelly was excused. Also present were Board
Attorney Steve Gross and Board Secretary Danielle Daskam.
II. AGENDA MODIFICATIONS
Claims for payment from Doris Darough and Rudy Peden were added to the
agenda.
III. APPROVAL OF MINUTES
A. Minutes of the April 1, 2014 LEOFF Board Meetinq
Motion by Member Holman, second by Member Vogel to approve 4he
minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 4-0
IV. UNFINISHED BUSINESS
A. Pendinq Disabilitv Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Pendinq Medical/HearinqNision Dental Claims
1. Rudv Peden
a. Request for reimbursement of inedical expenses in the amount
of $232.28 for services received on November 14, 2013 and
processed by insurance on November 26, 2013. On February
4, March 4, and April 1, 2014, the Board tabled Mr. Peden's
claim. No further information was received.
Motion by Member Osborne, second by Member Vogel tb table
the claim until the next meeting.

No request for reconsideration was received..
C. Medical/HearinqNision/Dental Claims
1. Doris Darouqh
Request for reimbursement of Medicare Part B premiums from the
years 2011 and 2012 and not previously claimed.
Finding to time limit for submission of claims related to
reimbursement of Medicare Part B premiums in the current
policies, Member Holman moved to approve the claim as
submitted. Motion was seconde.d by Member Vogel.
MOTION CARRIED UNANIMOUSLY. 4-0
2. Ed Henderson
a. Request for reimbursement of inedical expenses in the amount
of$121.27 for services received on an unknown date.
Motion by Member Vogel, second by Member Holman to table
the claim pending the receipt of the ins.urance and Medicare
explanation of benefits statement.
MOTION CARRIED UNANIMOUSLY. 4-0
3. William Pitzl
Request for reimbursement of dental expenses in the amount of
$299.70 for denfal services received on April 23, 2014. Mr. Pitzl
has not received dental expense reimbursement in 2014.
Motion by Member Osborne, second by Member Holman to
approve the claim in the amount of$299.70.

5. Loren Anderson
Request for reimbursement of dental expenses in the amount of
$821.75 for services received on March 24 and April 7, 2014. Mr.
Anderson has not received dental reimbursement in 2014.
Motion by Member Osborne, second by Member Vogel to approve
the claim in the amount of$821.75.
MOTION CARRIED UNANIMOUSLY. 4-0
6. Edward Sluvs
Request for reimbursement medical expenses in the amount of
$43.90 for services received January 16, February 16 and March
16, 2014.
Motion by Member Vogel, second by Member Osborne to table the
claim pending receipt of the insurance explanation of benefits
statement.
MOTION CARRIED UNANIMOUSLY. 4-0
7. Monroe Shropshire
Request for reimbursement of dental expenses in the amount of
$172.00 for services received on March 26, 2014. Mr. Shropshire
has not received dental reimbursement in 2014.

Request for reimbursement of long term/home health care c.osts in
the amount of$8,100.00 for the month of April 2014.
Motion by Member Osborne, second by Member Vogel to approve
the claim in the amount of $8,100.00.

9. Rudv Peden
a. Request for reimbursement of inedical expenses in the amount
of $1,118.70 for services received on January 28, 2014 and
processed by insurance on February 17, 2014..
Motion by Member Osborne, second by Member Vogel to
approve the claim in the amount of $1,118.70.
MOTION CARRIED UNANIMOUSLY. 4-0
b. Request for reimbursement of inedical expenses in the amount
of $248.60 for services received on March 11, 2014 and
processed by insurance on April 14, 2014.
Motion by Member Vogel, second by Member Osborne to
approve the claim in the amount of $248.60.
MOTION CARRIED UNANIMOUSLY. 4-0
c. Request reimbursement of inedical expenses in the amount of
$140.70 for medical services received on November 8, 2013.

D. Other Discussion
1. Chair Pro Tem Petersen noted there are a few retired Fire
members who are not enrolled in Medicare Part A, and thereby
not eligible for the Medicare Advaritage medical program. The
Board Secretary will confirm with Human Resources whether
those not eligible for Medicare Part A remain on Regence Plan A
medical plan.
2. Chair Pro Tem Petersen reported he sent out emails to all LEOFF
1 retirees regarding the premium increase notice from Allianz that
was mailed to all retirees. Allianz is asking the retiree to make a
choice regarding the changes. The City needs to respond to this
request on behalf of the retirees, not the retiree. The City is
responsible for the insurance premium, not the retiree.
VI. ADJOURNMENT
There being no further business to come before the Board, the meefing
adjourned at 10:22 a.m.
