pursuant to Chapter 60.04 RCW, do hereby certify that the charges set forth below against the property
described, constitute connection and /or service charges that are delinquent. Notice is hereby given that
additional delinquent service charges may continue to accrue after the date listed below and the claim of
lien herein shall apply to such additional delinquencies. Partial payments may be accepted and applied
