MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Council
Conference Room located on the first floor of Aubum Gity Hall, 25 West Main
Street in Auburn. Members present included Chair Kelly, Chair Pro Tem Bill
Petersen, Member Yolanda Trout, Member Claude DaCorsi, and Member-at-
large Kurt Vogel. Also present were Board Attorney Neil Wachfer and Board
Secretary Danielle Daskam.
II. AGENDA MODIFICATIONS
A claim from Doris Darough was added to the agenda.
III. APPROVAL OF MINUTES
A. Minutes of the Februarv 3, 2015 LEOFF Board Meetinq
Motion by Member Vogel, second by Ghair Pro Tem Petersen to approve
the minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 5-0
IV. UNFINISHED BUSINESS
A. Pendinq Disabilitv Leave/Retirement Aaalications
There was no pending disability leave/retirement application.
B. Pendinq Medical/Hearinq/Vision Dental Claims
There was no pending claim.
V. NEW BUSINESS
A. Disability LeavelRetirement Applications
No application for disability leave/retirement was received.
B. Appeals/Requests for Reconsideration
No request for reconsideration was received.

C. Medical/HearinqNision/Dental Claims
1. Rudv Peden
a. Request for reimbursement of inedical expenses in the amount
of $525.52 for services received on January 2, 2015
b. Request for reimbursement of inedical expenses in the amount
of $120.00 for services received October 20 and December 3,
2014.
Motion by Chair Pro Tem Petersen, second by Member Vogel to
table both claims pending the receipt of the insurance explanation
of benefits statements.
MOTION CARRIED UNANIMOUSLY. 5-0
2. Ed Henderson
Request for reimbursement of dental expenses in the amount of
$598.50 for services received on January 14, 2015. Mr.
Henderson's dental expenses were previously found to be
medically necessary.
Motion by Member Vogel, second by Chair Pro Tem Petersen to
approve the claim in the amount of $598.50.
MOTION CARRIED UNANIMOUSLY. 5-0
3. John Baker
Request for reimbursement of vision expenses in the amount of
$578.95 for services received on February 5, 2015. Mr. Baker has
not received vision expense reimbursement in the past year.
Motion by Chair Pro Tem Petersen, second by Member Trout to
approve up the policy annual mazimum of$270.00.
MOTION CARRIED UNANIMOUSLY. 5-0
4. Terrv M. Dunbar
Request for reimbursement of dental expenses in the amount of
$1,379.00 for services received on January 12, 27, and February
3, 2015. Mr. Dunbar has not received dental reimbursement in
2015.

Motion by Chair Pro Tem Petersen, second by Member Trout to
approve the claim in the amount of $1,200.00.
MOTION CARRIED UNANIMOUSLY. 5-0
5. David Jarqer
a. Request for reimbursement of vision expenses in the amount
of $305.00 for services received on February 4, 2015.
Motion by Member Vogel, second by Chair Pro Tem Petersen
to approve payment of policy annual maximum of$270.00.
MOTION CARRIED UNANIMOUSLY. 5-0
b. Request for pre-authorization for purchase and reimbursement
of lift chair in the amount of $2,174.88 less amount paid by
insurance, if any.
Motion by Chair Pro Tem Petersen, second by Member
DaCorsi to approve pre-authorization for purchase and
reimbursement of a lift chair less any Medicare or insurance
payments.
MOTION CARRIED UNANIMOUSLY. 5-0
6. Doris Darouqh
Request for reimbursement of long term care costs for the month
of February 2015 in the amount of $7,952.00 (28 days @ $284.00
per day).
Motion by Member Vogel, second by Chair Pro Tem Petersen to
approve the claim in the amount of$7,952.00.
MOTION CARRIED UNANIMOUSLY. 5-0
D. Other Discussion
1. Medicare Part D
Chair Pro Tem Petersen explained that Medicare eligible retirees
with a current Modified Adjusted Gross Income over $85,000
(individual) or $170,000 (married filing jointly) have to pay an
additional premium for both Medicare Part B and Medicare Part D.

Earlier Board action has authorized reimbursement of Medicare
Part B premiums upon submission of a claim and approval by a
LEOFF Board representative.
So far, two LEOFF 1 retirees have been affected by the new
Medicare Modified Adjust Gross Income threshold and are paying
Medicare Part D premiums. If or when the retiree's income level
falls below the income threshold, the requirement for payment of
additional Medicare premiums by the retiree should be eliminated.
Policy 8.02(F) relates to annual Medicare premium reimbursement
as does Policy 9.01(I). Chair Pro Tem Petersen recommended
the Board eliminate reference to any particular Medicare plan and
leave the reference "Medicare premium(s)."
Chair Kelly requested a LEOFF Policy and Procedure amendment
and the two claims from the affected retirees be placed on the
next LEOFF Board agenda for consideration.
VI. ADJOURNMENT
There being no further business to come before the Board, the meefing
adjoumed at 10:24 a.m.
