MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Council
Conference Room located on the first floor of Auburn City Hall, 25 West Main
Street in Auburn. Members present included Chair Kelly, Chair Pro Tem Bill
Petersen, Member Yolanda Trout, and Member-at-large Kurt Vogel. Member
Claude DaCorsi arrived at 10:04 a.m. Also present were Board Attorney Neil
Wachter and Board Secretary Danielle Daskam.
II. AGENDA MODIFICATIONS
Claims from Doris Darough, Ed Henderson, and Robert Almy were added to the
agenda.
III. APPROVAL OF MINUTES
A. Minutes of the March 3 2015 LEOFF Board Meetinq
Motion by Member Vogel, second by Chair Pro Tem Petersen to approve
the minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 4-0
IV. UNFINISHED BUSINESS
A. Pendinq Disabilitv Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Pendinq MedicallHearinqNision Dental Claims
1. Rudv Peden
a. Request for reimbursement of inedical expenses in the amount
of $525.52 for services received on January 2, 2015.
On March 3, 2015, the Board tabled Mr. Peden's claim pending
the receipt of insurance explanation of benefits statements.
Mr. Peden submitted additional information for the Board's
review.
Motion by Chair Pro Tem Petersen, second by Member Trout
to approve the claim in the amount of $525.52.

b. Request for reimbursement of inedical expenses in the amount
of $120.00 for services received October 20 and December 3,
2014.
On March 3, 2015, the Board tabled Mr. Peden's claim pending
the receipt of insurance explanation of benefits statements.
Since that time, Mr. Peden submitted an email dated March 6,
2015, requesting that his claim be withdrawn until further
information is provided to him from his provider and insurance.
Motion by Chair Pro Tem Petersen, second by Member Vogel
to accept Mr. Peden's request to withdraw his claim.
MOTION CARRIED UNANIMOUSLY. 4-0
V. NEW BUSINESS
A. Disabilitv Leave/Retirement Applications
No application for disability leave/retirement was received.
B. Appeals/Requests for Reconsideration
No request for reconsideration was received.
C. Medical/HearinqNision/Dental Claims
1. Robert Wiqlev
Request for reimbursement of dental expenses in the amount of
$160.00 for services received on March 10, 2015. Mr. Wigley has
not received dental expense reimbursement in 2015.
Motion by Chair Pro Tem Petersen, second by Member Vogel to
approve the claim in the amount of $160.00.
MOTION CARRIED UNANIMOUSLY. 4-0
2. Theodore Lindback
Request for reimbursement of dental expenses in the amount of
$428.45 for services received on February 25, 2015. Mr. Lindback
has not received dental expense reimbursement in 2015.
Motion by Member Vogel, s.econd by Chair Pro Tem Petersen to
approve the claim in the amount of $428.45.

3. Ed Wolters
a. Request for reimbursement of dental expenses in the amount
of $1,200.00. Mr. Wolters has not received dental expense
reimbursement in 2015.
Motion by Chair Pro Tem Petersen, second by Member Trout
to approve the claim in the amount of $1,200.00.
MOTION CARRIED UNANIMOUSLY. 5-0
b. Request for reimbursement of inedical expenses in the amount
of $57.08 for services received January 20, 2015.
Motion by Chair Pro Tem Petersen, second by Member Vogel
to approve the claim in the amount of $57.08.
MOTION CARRIED UNANIMOUSLY. 5-0
4. Cov Franklin Grall
Request for reimbursement of dental expenses in the amount of
$163.00 for services received on March 11, 2015. Mr. Grall has
not received dental expense reimbursement in 2015.
Motion by Member Vogel, second by Chair Pro Tem Petersen to
approve the claim in the amount of $163.00.

5. John Baker
Request for reimbursement of inedical expenses in the amount of
$72.00 for services received on February 9, 2015.
Motion by Chair Pro Tem Petersen, second by Member Vogel to
approve the claim in the amount of $72.00.

6. Doris Darouqh
Request for reimbursement of long term/home health care costs in
the amount of $8,804.00 (31 days $284/day) for the month of
March 2015.
Motion by Member Vogel, second by Chair Pro Tem Petersen to
approve the claim in the amount of $8,804.00.
MOTION CARRIED UNANIMOUSLY. 5-0
7. Ed Henderson
Request for reimbursement of dental expenses in the amount of
$242.25 for services received on February 17 and March 10, 2015.
Mr. Henderson's dental expenses were previously found to be
medically necessary.
Motion by Member Trout, second by Chair Pro Tem Petersen to
approve the claim in the amount of $24225.
MOTION CARRIED UNANIMOUSLY. 5-0
8. Robert Almv
Request for reimbursement of dental expenses in the amount of
$153.00 for services received on February 18, 2015. Mr. Almy has
not received dental expense reimbursement in 2015.
Motion by Member Trout, second by Chair Pro Tem Petersen to
approve the claim in the amount of $153.00.
MOTION CARRIED UNANIMOUSLY. 5-0
D. Other Discussion
1. Amendment of LEOFF Board Policy 9.01(I) to delete reference to
Medicare Part B:
I. All claims, except prescription claims and Social Security
� Medicare 1-'ar�-�--claims, shall be submitted to the Board
within ninety (90) days of date processed/paid by the
insurance documentation. The Board will review the
submitted claim at the next full Board meeting. If all
documentation is submitted properly the Board will consider
reimbursement of expenses. Fiowever, if for any reason, the

claim for paymenUreimbursement is insufficient, at the
discretion of 4he Board, it may table the claim until the next
meeting or until receipt of the requested documentation.
The claim may be tabled for three consecutive Board
meetings. If, after three (3) Board meetings the Claimant
has not provided the Board with the requested
documentation, the claim for paymenUreimbursement may
be rejected. (Rev. 5/2008)
Motion by Member Vogel, second by Member Trout to approve the
amendment to Section 9.01(I) of the LEOFF Boarii Policies and
Procedures as reflected above.
MOTION CARRIED UNANIMOUSLY. 5-0
As a result of the approval of the policy amendment above, the following
claims were considered:
Ron Loaan
Request for reimbursement of Medicare Part D premiums in the amount
of $373.20.
Motion by Chair Pro Tem Petersen, second by Member DaCorsi to
approve the claim in the amount of $373,20.
MOTION CARRIED UNANIMOUSLY. 5-0
John Dodd
Request for reimbursement of Medicare Part D premiums in the amount
of $145.20.
Motion by Member Vogel, second by Chair Pro Tem Petersen to approve
the claim in the amount of $145.20.
MOTION CARRIED UNANIMOUSLY. 5-0
2. Mav LEOFF Board Meetinq
It was consensus of the Board tb change the May 2015 LEOFF Board
meeting from Tuesday, May 5, 2015 to Monday, May 4, 2015 at 10:00
a.m. to accommodate the schedule of those members attending the
annual LEOFF Conference. The Board Secretary shall prepare the
necessary notice to change the meeting date for the month of May 2015.

VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
