MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Council
Conference Room located at Auburn City Hall, 25 West Main Street in Auburn.
Chair Kelly and the following Board members were present: Chair Pro Tem Bud
Larson (member-at-large), Member .Roger Thordarson, and Member Bill
Petersen. Member Lynn Norman was excused from attendance. Also present
were Board Attorney Joseph Beck and Board Secretary Danielle Daskam.
II. APPOINTMENT OF MEMBER AT LARGE
LEOFF Board Policies and Procedures provide that one member from the public
at large who resides within the city be appointed by the other four appointed
members for a two year term. Member Larson's term expired December 31,
2006.
Motion by Member Thordarson, second by Member Petersen to reappoint Bud
Larson as the member at large for a two year term expiring December 31, 2008.
MOTION CARRIED. 3-0. Member Larson abstained.
III. ELECTION OF CHAIRPERSON AND CHAIRPERSON PRO TEMPORE
Section 1.05 of the LEOFF Board Policies and Procedures provides that at the
first meeting of each year, the members shall elect from among the members, a
member to serve as Chairperson and a member to serve as Chairperson pro
tempore.
Motion by Member Thordarson, second by Member Petersen to nominate and
elect Jim Kelly as Chairperson and Bud Larson as Chairperson Pro Tem for the
year 2007.
MOTION CARRIED UNANIMOUSLY. 4-0.
IV. APPROVAL OF MINUTES
A. Minutes of the December 5, 2006 LEOFF Board Meeting
Motion by Chair Pro Tem Larson, second by Member Petersen to approve
the December 5, 2006 minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 4-0.

A. Pending Disability Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Medical/Hearing/Vision Claims
Request for reimbursement of dental expenses incurred on
November 9 and 15, 2006 in the amount of $679.00. (Mr. Petersen
has not been reimbursed for any previous dental expenses and his
claim is pending receipt of his dental insurance explanation of
benefits.)
Mr. Petersen's claim was tabled on December 5, 2006 pending
receipt of additional information from Mr. Petersen's dental
insurance provider. Mr. Petersen's explanation of benefits
statement was distributed during the meeting.
Motion by Chair Pro Tem Larson, second by Member Thordarson
to approve reimbursement of the balance after payment by Mr.
Petersen's insurance company in the amount of $167.30.
MOTION CARRIED. 3-0. Member Petersen abstained.
VI. NEW BUSINESS
A. Disability Leave/Retirement Applications
No new application for disability leave/retirement was received.
B. Appeals/Requests for Reconsideration
No appeal or request for .reconsideration was filed.
C. Medical/Hearing/Vision Claims
Dave Fugit
Request for reimbursement of dental expenses incurred on
December 5, 2006 in the amount of $668.00. (Mr. Fugit was
reimbursed for $532.00 in dental expenses on December 5, 2006.)

Motion by Member Petersen, second by Member Thordarson to
approve reimbursement of dental expenses in the amount of
$668.00.
MOTION CARRIED UNANIMOUSLY. 4-0.
2. Alan Kosko
Request for reimbursement of dental expenses incurred on
December 13, 2006 in the amount of $950.00. (Mr. Kosko was
reimbursed for $475.00 in dental expenses on November 7, 2006
and his current claim is pending receipt of his dental insurance
explanation of benefits statement.)

Motion by Member Petersen, second by Member Thordarson to
table the claim pending receipt of an explanation of benefits
statement from Mr. Kosko's dental insurance.

3. Bruce Caldwell
Request for reimbursement of medical expenses in the amount of
$98.80 per Regence Explanation of Benefits Statements
#0627232563800 and #0631932511900 for medical services
received on August 3 and November 8, 2006 and processed by
insurance on October 11 and November 29, 2006.
Motion by Member Petersen, second by Chair Pro Tem Larson to
approve reimbursement in the amount of $98.80.
MOTION CARRIED UNANIMOUSLY. 4-0.
4. James Fisk
Request for reimbursement of dental expenses incurred on
December 9, 2006 in the amount of $234.00. (Mr. Fisk has not
received reimbursement for dental expenses in 2006.)
Motion by Member Petersen, second by Chair Pro Tem Larson to
approve reimbursement of 2006 dental expenses in the amount of
$234.00.

5. John Calkins
a. Request for reimbursement of medical expenses in the
amount of $100.00 per Regence Explanation of Benefits
Statement #0531932391700 for medical services received
on October 21, 2005 and processed by insurance on
November 30, 2005.
Motion by Member Petersen, second by Chair Pro Tem
Larson to approve reimbursement in the amount of $100.00.
Chair Kelly noted that the claim is over one year old, and it is
at the discretion of the Board to consider the claim.
MOTION CARRIED UNANIMOUSLY. 4-0.
