MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to orde�at 10:00 a.m. in the Council
Conference Room located on the first floor of Auburn City Hall, 25 West Main
Street in Auburn. Members present included Chair Kelly, Chair Pro Tem Bill
Petersen, Member Yolanda Trout-Manuel, Member Claude DaCorsi and
Member-at-large David Cook. Also present were Board Attorney Doug Ruth and
Board Secretary Danieile Daskam.
II. AGENDA MODIFICATIONS
There was no change to the agenda.
III. APPROVAL OF MINUTES
A. Minutes of the June 7. 2016 LEOFF Board Meetinq
Motion by Chair Pro Tem Petersen, second by Member Trout-Manuel to
approve the minutes as distributed.
MOTION CARRIED UNANIMOUSLY 5-0
IV UNFINISHED BUSINESS
A. Pendinq Disabilitv Leave/Retiremerit Applications
There was no pending disability leave/retirement application.
B Pendin4 Medical/HearinaNision/Dental Claims
1 Bruce Caldwell
Request for reimbursement of expenses in the amount of $48.88
for purchase on January 13, 2016.
On April 5, May 3 and June 7, 2016, the Board tabled Mr
Caldwell's claim pending the receipt of a description and
explanation for the purchase. No new information has been
received.
Motion by Chair Pro Tem Petersen, second by Member Trout-
Manuel to deny the claim due to lack of information.

A. Disabilitv Leave/Retirement Aaalications
1 Rudv Peden
Effective Date of Disability June 13, 2016
Last Day of Service: June 11, 2016
On June 13, 2016, LEOFF Board Chair Kelly tentatively approved
disability leave for Rudy Peden effective June 13, 2016 with last
day of service on June 11, 2016, subject to full Board review and
approval. Mr Peden's application for disability leave/retirement
includes physician statements from Dr Ryder Gwinn and Jordan
Steed, PA-C dated June 10, 2016.
LEOFF BOARD ACTION: Board consideration of an Application
for Disability Leave or Retirement filed June 13, 2016.
Motion by Chair Pro Tem Petersen, second by Member Cook to
approve disability leave for Rudy Peden effective June 13, 2016.
MOTION CARRIED UNANIMOUSLY 5-0
B Appeals/Repuests for Reconsideration
No request for reconsideration was received.
C. Medical/HearinQNision/Dental Claims
1 Dave Schuur
Request for approval/reimbursement of long-term care expenses
for the month of June 2016 in the amount of$5,460.00
Motion by Chair Pro Tem Petersen, second by Member DaCorsi to
approve the claim in the amount of$5,460.00.
MOTION CARRIED UNANIMOUSLY 5-0
2. Cali Ann Edwards
Request for reimbursement of skilled nursing care in the amount of
$6,948.16
Motion by Member Trout-Manuei, second by Member Cook to
approve the claim.

Motion by Member DaCorsi, second by Chair Pro Tem Petersen to
amend the motion to pay up fo the Allianz daily rate ($298.00 per
day)
MOTION TO AMEND CARRIED UNANIMOUSLY 5-0
MOTION TO APPROVE THE CLAIM TO PAY LONG TERM CARE
FOR 21 DAYS AT THE ALLIANZ DAILY RATE ($298.00 per day).
MOTION CARRIED UNANIMOUSLY 5-0
3. Monroe Shroashire
Request for reimbursement of dental expenses in the amount of
$163.40 for services received on May 4, 2016 Mr Shropshire's
dental expenses have been previously found to be medically
necessary
Motion by Chair Pro Tem Petersen, second by Member Trout-
Manuel to approve the claim in the amount of$163.40
MOTION CARRIED UNANIMOUSLY 5-0
4 Stan Laatsch
a. Request for reimbursement of pharmacy expenses from
2013 in the amount of$171.91
b Request for reimbursement of pharmacy expenses from
2014 in the amount of$156 56
Motion by Chair Pro Tem Petersen, second by Member DaCorsi to
deny the request for reimbursement of 2013 and 2014 pharmacy
expenses as the expenses are outside the 12-month period for
submitting requests for reimbursement.
MOTION CARRIED UNANIMOUSLY 5-0
5. Loren Anderson
Request for reimbursement of dental expenses in the amount of
$1,200 00 for services received on May 9, 16, and 24, 2016. Mr
Anderson has not received dental expense reimbursement in 2016.
Motion by Chair Pro Tem Petersen, second by Member Cook to
approve the claim in the amount of$1,200 00

6. Ed.Henderson
Request for reimbursement of dental expenses in the amount of
$641.25 for services received on May 31, 2016. Mr Henderson's
dental expenses have been previously found to be medically
necessary
Motion by Chair Pro Tem Petersen, second by Member Cook to
approve the claim in the amount of$641.25.
MOTION CARRIED UNANIMOUSLY 5-0
7 Richard Schneider
Request for reimbursemerit of inedical expense capay in the
amount of $20.00 for services received on June 16, 2016.
Motion by Member Trout-Manuel, second by Chair Pro Tem
Petersen to approve the claim in the amount of $20.00.
MOTION CARRIED UNANIMOUSLY 5-0
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
