MINUTES
I. CALL TO ORDER
Member Roger Thordarson presided over the meeting due to the excused
absences of Chair Jim Kelly and Chair Pro Tem Bud Larson. Member
Thordarson called the meeting to order at 10:00 a.m. and the following members
were present: Member Thordarson, Member Norman, and Member Petersen.
Also present were Board Attorney Joseph Beck and Board Secretary Danielle
Daskam.
II. APPROVAL OF MINUTES
A. Minutes of the January 2, 2007 LEOFF Board Meeting
Motion by Member Petersen, second by Member Norman to approve the
January 2, 2007 minutes as distributed.
MOTION CARRIED UNANIMOUSLY: 3-0.
III. UNFINISHED BUSINESS
A. Pending Disability Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Medical/Hearing/Vision Claims
Alan Kosko
Request for reimbursement of dental expenses incurred on
December 13, 2006 in the amount of $950.00. (Mr. Kosko was
reimbursed for $475.00 in dental expenses on November 7, 2006
and his current claim is pending receipt of his dental insurance
explanation of benefits statement.) The claim was tabled on
January 2, 2007 pending receipt of the dental explanation of
benefits statement. On January 8, 2007, Mr. Kosko submitted his
explanation of benefits statement from Washington Dental Service.

Motion by Member Petersen, second by Member Norman to approve
payment in the amount of $475.00.
MOTION CARRIED UNANIMOUSLY. 3-0.
IV. NEW BUSINESS
A. Disability Leave/Retirement Applications
No new application for disability leave/retirement was received.
B. Appeals/Requests for Reconsideration
No appeal or request for reconsideration was filed.
C. Medical/Hearing/Vision Claims
1. Stan Laatsch
Request for reimbursement of medical expenses in the amount of
$500.00 per Regence Explanation of Benefits Statement
#0635532598900 for services received on December 20, 2006 and
processed by insurance on January 10, 2007.
Motion by Member Petersen, second by Member Norman to
approve the claim in the amount of $500.00.
MOTION CARRIED UNANIMOUSLY. 3-0.
2. Jim Kelly
Request for reimbursement of medical expenses in the amount of
$10.00 per Regence Explanation of Benefits Statement
#0701131543700 for services received on January 4, 2007 and
processed by insurance on January 17, 2007.
Motion by Member Petersen, second by Member Norman to
approve the claim in the amount of $10.00.
MOTION CARRIED UNANIMOUSLY. 3-0.
D. Correspondence
1. Letter dated February 2, 2007 addressed to LEOFF Board Chair
Jim Kelly from John T. Calkins.
The Board reviewed Mr. Calkins' letter in which he describes dental
work required as a result of radiation treatment for throat cancer.

Mr. Calkins indicates that he has exhausted his dental insurance
funds. He requests the Board's consideration of a claim for
reimbursement of the dental expenses in the amount of $1,246.00.
Board members requested that the Board Secretary advise Mr.
Calkins to submit a claim for the dental expenses and further
advise Mr. Calkins that the annual dental benefit is $1,200; any
reimbursement over and above the annual dental benefit would
require proof of medical necessity.
V. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:13 a.m.
