MIPIUTES
I. CALL- TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Legal Conference
Room located on the first floor of Auburn City Hall, 25 West Main Street in
Auburn. Members present included Chair Kelly, Member Yolanda Trout-Manuel,
Member Bob Baggett and Member-at-large David Cook. Also present were
Board Attorney Doug Ruth and Deputy City Clerk Shawn Campbell. Chair Pro
Tem Bill Petersen was excused.
II. AGENDA MODIFICATIONS
Claims from Theodore Lindback, Gali Ann Edwards, Coy Franklin Grall and
Michael Corkins were added to the agenda.
111. APPROVAL OF MINUTES
A. Minutes of the July 5, 2016 LEOFF Board_Meetinq
Motion by Member Trout-Manuel, second by Member Cook to approve the
minutes as distributed. .
MOTION CARRIED UNANIMOUSLY. 4-0
IV. UNFINISHED BUSIiVESS
A. Pendinq Disability Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Pendinq Medical/Hearing/Vision/Dental Claims
There was no pending claim.
V. NEW BUSINESS
A. Disability Leave/Retirement Applieations
1. Rudv Peden
Effective Date of Disability: June 13, 2016
Last Day of Service: June 11, 2016
On July 5, 2016, the LEOFF Board approved disability leave for
Rudy Peden effective June 13, 2016. Mr. Peden submitted his
monthly physician's report from Dr. Ryder Gwinn and received by
the Board Secretary on Jwly 12, 2016.

LEOFF BOARD ACTION: Board consideration of a continuation of
Disability Leave filed June 13, 2016.
Motion by Member Trout-Manuel, second by Member Cook to
continue disability leave for Rudy Peden.
Chair Kelly noted Mr. Peden is required to submit a letter from his
� physician monthly to continue disability leave.
MOTION CARRIED UNANIMOUSLY. 4-0
B. Apq.eals/Requests for Reconsideration
No request for reconsideration was received.
C. Medical/Hearing/Vision/Dental Claims
1. Dave Schuur
Request for approval/reimbursement of long-term care expenses
for the month of July 2016 in the amount of$5,460.00.
Motion by Member Trout-Manuel, second by Member Cook to
approve the claim in the amount of$5,460.00.
Ghair Kelly noted the bill shows Mr. Schuur is in a private room.
The policy pays for a semi private room unless a private room is
medically necessary. The Chair directed the secretary to send a
letter asking for the medical necessity of the private room
MOTION CARRIED UNANIMOUSLY. 4-0
2. Don Webb
a. Request for reimbursement of dental expenses in the
amount of$300.00 for services received on June 30, 2016.
b. Request for reimbursement of dental expenses in the
amount of$150.00 for services received on July 14, 2016.
Motion by Member Cook, second by Member Trout-Manuel to
approve the claims in the amount of$450.00.

Request for reimbursement of vision exam expenses in the amount
$65.00 for services received on May 24, 2016.
Motion by Member Trout-Manuel, second by Member Baggett to
approve the claim in the amount of$65.00.
MOTION CARRIED UNANIMOUSLY. 4-0
4. Bruce Caldwell
Request for reimbursement of hospice/medical care in the amount
of$630.00 for services received on May 5-6, 2016.
Motion by Member Trout-Manuel, second by Member Baggett to
table the claim in fhe amount of$630.00. The committee requested
the secretary send a letter requesting the insurance Explanation of
Benefits.
MOTION CARRIED UNANIMOUSLY. 4-0
5. Theodore Lindback
Request for reimbursement of Medicare Premium in the amount of
$365.40.
Motion by Member Cook, second by Member Baggett to approve
the claim in the amount of$365.40.
MOTION CARRIED UNANIMOUSLY. 4-0.
6. Cali Ann Edwards
Request for reimbursement of skilled nursing care in the amount of
$390.00
Chair Kelly noted the claim is for the remainder of the bill submitted
in July for skilled nursing care. The Board paid up to the Allianz
daily rate.
Motion by Member Cook, second by Member Baggett to approve
the claim in the amount of$390.00.

Request for reimbursement of dental expenses in the amount of
$301.15 for services received on June 28, 2016.
Motion by Member Cook, second by Member Trout-1lllanual to
approve the claim in the amount of$301.15.
MOTION CARRIED UNANIMOUSLY. 4-0
8. Michael Corkins
Request for approval of hydro-therapy walk in tub in the amount of
$15,000.00
Motion by Member Baggett, second by Member Trout-Manual to
table the claim in the amount of $15,000.00 and ask the member
for letters from physicians regarding medical necessity.
MOTION CARRIED UNANIMOUSLY. 4-0
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:25 a.m.
