MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Legal Conference
Room located on the first floor of Auburn City Hall, 25 West Main Street in
Auburn. Members present included Chair Kelly, Chair Pro Tem Bill Petersen,
Member Yolanda Trout-Manuel, Member Bob Baggett, and Member-at-large
David Cook. Also present were Board Attorney Doug Ruth and Board Secretary
Danielle Daskam.
II. AGENDA MODIFICATIOIVS
Claims from Stan Laatsch were added to the agenda.
I11. APPROVAL OF MINUTES
A. Minutes of the Auqust 2, 2016 LEOFF Boacd Meetinq
Motion by Chair Pro Tem Petersen, second by Member Trout-Manuel to
approve the minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 5-0
IV. UIVFIIVISHED BUSINESS
A. Pendinq Disabilitv Leave/Retirement Applications
1. Rudv Peden
Effective Date of Disability: June 13, 2016
Last Day of Service: June 11, 2016
On July 5, 2016, the LEOFF Board approved disability leave for
Rudy Peden effective June 13, 2016. Mr. Peden has submitted the
enclosed physician's statement and request to return to work
without restriction effective August 27, 2016.
On August 17, 2016, Chair Kelly approved Mr. Peden's return to
full and unrestricted duty effective August 27, 2016, pending the
Board's approval.
Motion by Chair Pro Tem Petersen, second by Member Trout-
Manuel to confirm the Chair's action and approve Mr, Peden's
return to full and unrestricted duty effective August 27, 2016.

B. PendinQ Medical/Hearing/VisionLDental Claims
1. Bruce Caldwell
Request for reimbursement of hospice/medical care in the amount
of $630.00 for services received on May 5-6, 2016: On August 2,
2016, the Board tabled the claim for Mr. Caldwell pending the
receipt of an insurance explanation of benefits statement. No new
information was received.
Motion by Member Cook, second by Member Baggett to table the
claim for an additional month pending the receipt of additional
information.
MOTION CARRIED UNANIMOUSLY. 5-0 '

2. Michael Corkins
Request for approval of hydro-therapy walk in tub in the amount of
$15,000.00. On August 2, 2016, the Board tabled Mr. Corki_r�s'
claim pendi.ng the receipt of a statement from Mr: Corkins'
physician swpporting the medical necessity of the equipment. No
new information was received.
Motion by Member Trout-Manuel, second by Chair Pro Tem
Petersen to table the claim for an additional month pending the
receipt of additional information.

approve payment in the amount of$5,460.00.
MOTION CARRIED UNANIMOUSLY. 5-0
2. Michael Darbv
Request for reimbursement of dental expenses in the amount of
$243.00 for services received on July 28, 2016. Mr. Darby has not
received dental reimbursement in 2016.
Motion by Member Cook, second by Chair Pro Tem Petersen to
approve the claim in the amount of$243.00.
MOTION CARRIED UNANIMOUSLY. 5-0
3. Dale Volstad
Request for reimbursement of inedical expenses in the amount
$163.00 for services received on January 8, 2016.
Motion by Chair Pro Tem Petersen, second by Member Trout-
IVlanuel to table fhe claim pending the receipt of an explanation
from the Human Resources Department for insurance's non- ,
payment of the claim.
MQTION CARRIED UNANIMOUSLY. 5-0
4. Victor Shively
a. Request for reimbursement of inedical expenses in the amount
of$75.00 for services received on June 22; 2016.
Motion by Chair Pro Tem Petersen, second by Member Trout-
Manuel to table the claim pending receipt of an insurance
explanation of benefits statement.

b. Request for reimbursement of vision exam expenses in the
amount of$69.00 for services received on August 18, 2016.
Motion by Chair Pro Tem Petersen, second by Member Cook to
approve the claim in the amount of$69.00.

Request for reimbu.rsement of dental expenses in the amount of
$79:00 for services received on August 19, 2016. Mr. Board has
no# received dental expense reimbursement in 2016.
Motion by Chair Pro Tem Petersen, second by Member Baggett to
approve the claim in the amdunt of$79.00.

6. Stan Laatsch
a. Request for reimbursement of dental expenses in the amount of
$125,00 for services received on August 31; 2016., Mr. Laatsch
has not received dental expense reimbursement in 2016.
Motion by Member Trout-Manuel, second by Chair Pro Tem
Petersen to approve the claim 'in the amount of$125.00.

$219.98 f�r ,services re�e�ved on November 5, 2015. Mr,
Laa#sch has not received vision expense reimbursement in the
last 12 months.

to approve the claim in t�� amount of$219.98.
MOTION CARRIED UNANIMOUSLY. 5-0
c. Request for reimbursement of vision expenses in the amount of
$189.98 for senrices received on July 29, 2016.
Motion by Member Trout-Manuel, second �y Chair Pro Tem
Petersen to approye the claim in the amoun# of$189,.98.
Chair Pro Tem Petersen questioned the purpose.of the seaond
pair of the g{asses. _. Chair Kelly note� the claim did not
articulate the type of glas�es other than bi-focal.

D. Other Discussion
Chair Kelly announced he will be out of town and unable to attend the
next LEOFF Board meeting.
