MIiVUTES
I. CALL TO ORDER
Chair Pro Tem Bill Petersen ealled the meeting to order at 10:00 a.m. in
Conference Room 2 located on the second floor of Auburn City Hall, 25 West.
Main Street in Auburn. Members present ineluded Chair Pro Tem Bill Petersen,
Member Bob Baggett, and Member-at-Iarge David Cook. Chair Jim Kelly and
Member Yolanda Trout-Manuel were excused. Also present were Board
Attorney Doug Ruth and Board Secretary Danielle Daskam.
II. AGENDA MODIFICATIONS
Glaims for Helen Bowden and Monroe Shropshire were added to the agenda.
III. APPROVAL OF nAINUTES
A. Minutes of the September 6 2016 LEOFf Board Meeting
Motion by Member Cook, second by Member Baggett to approve the
minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 3-0
IV. UNFINISHED BUSIIVESS
A. Pendinq Disabilitv Leave/Retirement Applications
There was no pending disability leave/retirement application.

1. Bruce Caldwell
Request for reimbursement of hospice/medical care in the amount
of $630.00 for services received on May 5-6, 2016. On August 2
and September 6, 2016, the Board tabled the claim for Mr, Caldwell
pending the receipt of an insurance explanation of bene�ts
statement. No new information was received.
Motion by Member Cook, second by Member Baggett to table the
claim until the next meeting.

2. 1lllichael Corkins
Request for approval of hydro-therapy walk in tub in the amount of
$15,000.00. On August 2 and September 6, 2016, the Board
tabled Mr. Corkins' claim pending the receipt of a statement from
Mr. Corkins' physician supporting the medical necessity of the
equipment. No new information was received.
Mofion by Member Cook, second by Member Baggett to table the
claim until the next meeting.
MOTION CARRIED UNANIMOUSLY. 3-0
3. bale Volstad
Request for reimbursement of inedical expenses in the amount
$163.00 for services reeeived on January 8, 2016. On September
6, 2016, the LEOFF Board tabled the claim pending receipt of an
� explanation from the Human Resources Department and the
i.nsurance representative for the non-payment of the claim by
insurance.
The LEOFF Board Secretary reported Human Resources staff
learned from the insuranee broker that the claim is a covered
expense by insurance. The claim will be resubmitted to insurance
and the provider paid. Mr. Volstad already personally paid the
claim, and he will be advised to contact his provider for any
reimbursement.
Motion by IVlember Baggett, second by Member Cook to deny the
claim.
MOTION CARRIED UNANIMOUSLY. 3-0
4. �ctor Shivelv
Request for reimbursement of inedical expenses in the amount of
$75.00 for services received on June 22, 2016. On September 6,
2016, the LEOFF Board tabled the claim pending the receipt of an
explanation of benefits statement. No new information has been
received.
Motion by Member Cook; second by Member Baggett to table the
claim for an additional month.

The LEOFF Board Secretary reported Mr. Shively's provider does
not bill insurance. Mr. Shively has been provided with a Medicare
request for payment form and instructions. Mr. Shively has been
advised to complete the Medicare form and submit his claim to the
Medicare office listed for his state of residence.
MOTION CARRIED UNANIMOUSLY. 3-0
5. Stan Laatsch
Request for reimbursement of vision expenses in the amount of
$189.98 for services received on July 29, 2016. On September 6,
2016, the Board tabled Mr. Laatsch's request for reimbursement of
a second pair of glasses within a twelve month period until the
receipt of additional information from Mr. Laatsch on the type and
purpose of the second pair of glasses.
The LEOFF Board Secretary reparte.d that Mr. Laatsch described
the second pair of glasses as sunglasses. According to the LEOFF
Board Secrefary, Mr. Laatsch was unaware of the Board policy for
reimbursement of vision expenses on a 12 eonsecutive month
basis; Mr. Laatsch assumed reimbursement was on a calendar
year basis. The LEOFF Board Secretary reported that Mr. Laatsch
would not pursue the claim for the second pair of glasses any
further.
Motion by Member Baggett, second by Member Cook to close the
claim.
MOTION CARRIED UNANIMOUSLY. 3-0

1. Dave Schuur .
a. Request for approval/reimbursement of long-term care
expenses for the month of September 2016 in the amount of
$5,460.00.
Motion by Member Cook, second by Member Baggett to
approve the claim in the amount of$5,460.00.
Chair Pro Tem Petersen acknowledged the receipt of a
physician's statement explaining the need for a private room for
Mr. Schuur.
MOTION CARRIED UNANIIVIOUSLY. 3-0
b. Request for reimbursement of inedical expenses in the amount
of$52.63 for services received on July 26, 2016.
Motion by Member Cook, second by Member Baggett to
approve the claim in the amount of$52.63.
MOTION CARRIED UNANIMOUSLY. 3-0
2. Cali Ann Edwards
Request for reimbursement of equipment in the amount of ,
$20,077.35 (This request was originally submitted at the January
5, 2016 LEOFF Board meeting.)
Motion by Member Cook, second by Member Baggett to deny the
claim.
Member Cook noted the physician's statement states the
equipment "to be very beneficial� but does not state the equipment
is medically necessary.
Board Attorney Ruth stated the Board has the option to require an
independent examination,
Motion by Member Cook, second by Member Baggett to table the
claim pending fhe receipt of additional information supporting the
medical necessity of the equipment.

table the claim for an additional month pending the receipt of
proof of inedical necessity.

4. Harold Philbin
Request for reimbursement of hearing aid expenses in the amount
of $1,299.99 for one hearing aid purchased from the Board's
preferred provider, Costco Hearing Aid Center, on August 30,
2016. Mr. Philbin was last approved for a hearing aid in August
2011.
Motion by Member Cook, second by Member Baggett to approve
the claim as submitted.

Request for reimbursement of dental expenses in the amount of
$29.00 for services received on August 22, 2016.

has not received dental expense reimbursement in 2016.

7. Monroe Shropshire
Request for reimbursement of dental expenses in the amount of
$163.40 for services received on September 14, 2016. Mr.
Shropshire's dental expenses were previously found ta be
medically necessary.
Motion by Member Baggett, second by Member Cook to approve
the claim in the amount of$163.40.
MOTION CARRIED UNANIMOUSLY. 3-0
8. Helen J. Bowden
Request for reimbursement of dental expenses in the amount of
$321,10 for services received on August 3, 2016. Ms. Bowden has
not received dental expense reimbursement in 2016.
Motion by IUlember Cook, second by Member Baggett to approve
the claim in the amount of$321.10.

a. Fire LEOFF Representative Election
Board members reviewed the Fire LEOFF Representative election
schedule. Request for nominations will be mailed to fhe members
on October 17, 2016.

adjourned at 10:19 a.m.
