NiINUTES
I. CALL TO ORDER �
Ghair Jim Kelly called the meeting to order at 10:00 a.m. in Conference Room 2
located on the second floor of Auburn City Hall, 25 West Main Street in Auburn.
Members present included: Chair Kelly, Chair Pro Tem Bill Petersen, Member
Yolanda Trout-Manuel, Member Bob Baggett, and Member-at-large David Cook.
Also present were Board Attorney Doug Ruth and Board Secretary Danielle
Daskam.
II. AGEiVDA MODIFICATIOMS
Claims from Stan Laatsch and Richard Schneider were added to the agenda.
III. APPROVAL OF flAINUTES
A. Minutes of the October 4,20_1_6:LEOFF Board Meetinq
Motion by Member Trout-Manuel, second by Member Cook to approve the
minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 5-0
IV. UIVFINISHED BUSINESS
A. Pending Disabilitv Leave/Retirement Applications
There was no pending disability leave/retirement application.
B: Pendinq Medical/HearinqNision/Dental Claims
1. Bruce Caldwell
Request for reimbursement of hospice/medical care in the amount
of $630.00 for services received on May 5-6, 2016. On August 2,
September 6 and October 4, 2016, the Board tabled the claim for
Mr. Caldwell pending the receipt of an insurance explanation of
benefits statement. No new information was received.
- Motion by Chair Pro Tem Petersen, second by Member Cook to
deny the claim due to lack of information and response to the
Board's request.

2. Michael Corkins
Request for approval of hydro-therapy walk in tub in the amount of
$15,000.00. On August 2, September 6 and October 4, 2016, the
Board tabled Mr: Corkins' claim pending the receipt of a statement
from Mr. Corkins' physician supporting the medical necessity of the
equipment. Mr. Corkins submitted additional documents in
response to the Board's request.
Motion by Chair Pro Tem Petersen, second by Member Baggett to
approve the claim,
MOTION FAILED. 0-5
Motion by Chair Pro Tem Petersen, second by Member Cook to
submit Mr. Corkins for an independent physician examination in
order to determine whether the equipment is medically necessary
versus medically benefcial.
MOTION CARRIED UNANIMOUSLY. 5-0
4. Victoc Shivelv
Request for reimbursement of inedical expenses in the amount of
$75.00 for services received on June 22, 2016. On September 6
and October 4, 2016, the LEOFF Board tabled the claim pending
the receipt of an explanation of benefts statement. No new
information was received.
Motion by Chair Pro Tem Petersen, second by Member Trout-
Manuel to table the claim for an additional month.
MOTION CARRIED UNANIMOUSLY. 5-0
5. Cali Ann Edwards
Request for reimbursement of equipment in the amount of
$20,077.35 (This request was originally submitted at the January
5, 2016 LEOFF Board meeting.) On October 4, 2016, the Board
tabled Ms. Edwards' claim pending the receipt of additional
information supporting the medical necessity of the equipment. No
new information was received.
Motion by Chair Pro Tem Petersen, second by Member Baggett to
table the claim for an additional month.

6. Robert Wiqlev
Request for pre-authorization of dental expenses in the amount of
$10,214_00. On October 4, 2016, the LEOFF Board tabled the
claim pending receipt of proof of inedical necessity for the dental
work. No new information was received.
Motion by Member Trout-Manuel, second by Chair Pro Tem
Petersen to table the claim for an additional month.
MOTION CARRIED UNANIMOUSLY. 5-0

No new application for disability leave was received.
B. Apqeals/Requests for Reconsideration
No request for reconsideration was received.
C. Medical/Hearinq/Vision[Dental Claims
1. Dave Schuur
a. Request for approval/reimbursement of long-term care
expenses for the month of October 2016 in the amount of
$5,460.00.
Motion by Chair Pro Tem Petersen, second by Member Trout-
Manuel to approve the claim in the amount of$5,460.00.
MOTION CARRIED UNANIMOUSLY. 5-0
b. Request for reimbursement of inedical expenses in the amount
of$169.43 for services received on February 13, 2016.
Motion by Member Cook, second by Chair Pro Tem Petersen to
approve the claim in the amount of$169.43.

Request for approval of long-term care/home health care in the
amount of$488.10 for the period October 5 to October 14, 2016.
Motion by Chair Pro Tem Petersen, second by Member Baggett to
approve the claim in the amount of$488.10.
MOTION CARRIED UNANIMOUSLY. 5-0
3. M. Eric Bearscove
Request for reimbursement of vision exam expenses in the amount
of $110.00 and lenses and frames expenses in the amount of
$219.98 for services received on October 17, 2016. Mr. Bearscove
received vision expense reimbursement in the amount of $270.00
on January 5, 2016 for services received on October 28, 2015.
Motion by Member Baggett, second by Member Cook to approve
the claim in the amount of $110.00 for exam serviees and $219.98
for lenses and frames.
MOTION CARRIED UNANIMOUSLY. 5-0
4. Ed Wilson
Request for reimbursement of inedieal expenses in the amount of
$1,812.19 for services received January 13, Mlarch 30, July 25,
August 4.
Motion by Member Cook, second by Chair Pro Tem Petersen to
approve the claim.
Upon further discussion and review, Member Cook moved and
Member Trout-Manuel seconded to table the claim pending the
receipt of Medicare/insurance explanation of benefits statements.

has not received dental expense reimbursement in 2016.
