CALL TO ORDER Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Conference Room 2 located on the second floor of Auburn City Fiall, 25 West Main St�eet in Auburn. Members present included Chair Kelly, Chair Pro Tem B'ill Petersen, Member Yolanda Trout-Manuel, Member Bob Baggett, and Member-at-large David Cook. Also present were Board Attomey Doug Ruth and Board Secretary Danielle Daskam. II. ELECTION OF CHAIRPERSON AND CHAIRPERSON PRO TEMPORE. _ Section 1.05 of the LEOFF Board Policies and Procedures provides that at the first meeting of each year, the members shall elect from among the members, a member to serve as Chairperson and a member to serve as Chairperson pro tempore. Motion by Chair Pro Tem Petersen, second by Member Trout-Manuel to nominate and elect Jim Kelly as Chair of the LEOFF Board for the year 2017. MOTION CARRIED UNANIMOUSLY. 5-0 Motion by Member Cook, second by Member Trout-Manuel to nominate and elect Bill Petersen as Chair Pro Tem for the year 2017. MOTION CARRIED UNANIMOUSLY. 5-0 III. AGENDA MODIFICATIONS There was no addition to the agenda. IV. APPROVAL OF MINUTES A. Minutes of the December 6. 2016 LEOFF Board Meetina Motion by Chair Pro Tem Petersen, second by Member Baggett to approve the minutes at distributed. MOTION CARRIED UNANIMOUSLY. 5-0. V... Pendinq Medical/HearinaNision/Dental Claims 1. Michael Corkins Request for approval of hydro-therapy walk in tub in the amount of $15,000.00. On August 2, September 6 and October 4, 2016, the Board tabled Mr. Corkins' claim pending the receipt of a statement from Mr. Corkins` physician supporting the medica.l necessiry of the equipment. On November 1, 2016, Mr. Corkins submitted additional information in response to the Board's request, and the Board subsequently refeRed Mr. Corkins to an independent medical examination (IME). It was noted that Mr. Corkins' IME is delayed until further nofice due to other med.ical conditions. 2. Cali Ann Edwards Request for reimbu�sement of equipment in the amount of $20,077.35 (This request was originally submitted at the Janua_ry 5, 2016 LEOFF Board meeting.) On October 4, November 1 and December 6, 2016, the Board tabled Ms. Edwards' claim pending the receipt of additional information supporting the med'ical necessity of the equipment. No new information was received. Motion by Chair Pro Tem Petersen, second by Member Cook to deny the claim due to lack of information. MOTION CARRIED UNANIMOUSLY. 5-0 3. Robert Wiqlev Request for pre-authorization of dental expenses in the amount of $10,214.00. On October 4, November 1 and December 6, 2016, the LEOFF Board tabled the claim pending receipY of pro.of of medical necessity for 4he dental work. No new information was received. Motion by Chair Pro Tem Petersen, second by Member Trout- Manuel to deny the claim due to lack of information. MOTION CARRIED UNANIMOUSLY. 5-0 4. Ed Wilson Request for reimbursement of inedical e.xpenses in the amount of $1,812.19 for services received January 13, March 30, July 25, August 4.
