REGULAR MEETING DULY ASSEMBLED, HEREWITH RESOLVES THAT: Section L The Mayor and City Clerk of the City of Aubum are herewith authorized to execute a Mea1s on Wheels Satellite Site Memorandum of Agreement for 2002 between the City of Auburn and Senior Services of Seattle/King Connty (SSS/KC) to provide home delivered meal service in the Auburn azea to the elderly who are homebound oi unable to prepare balanced meals. A copy of said Agreement is aftached hereto and denominated Exhibit``A" and incorporated herein. Section 2. Heid, City Attomey Resolution No.3418 Fetiniary 1,2002 Page�2�of 2 MEMOlaANDUM OF AG1�Ell�IENT Meals on Wheels Satellite Site: Auburn This Memorandum of Agreement is between Senior Services of Seattle/King County Meals on Wheels Program and the City of Anbuin. In recognirion of the need to pmvide home-delivered meal service to the elderly in the Aubum azea through the use of senior center facilities and volunteer delivery staff, and in recognirion that Senior Services has been granted funds to contribute to the cost of such acriviries, it is hereby agreed that Aubum Senior Center and Senior Services Meals on Wheels Program will work together to achieve these ends for the greater good of the Aubum community. 1. It is agreed that this satellite home-delivered meal distribution site shall be located at the Anburn Senior Center, 808 Ninth SE, Aubum, Wasiiingtbn 98002. 2. The program will supply two freezers foi storage of the meals as needed. The site Meals on Wheels coordinator is responsible for pioper maintenance of the freezers, including defrosting and keeping them free of rion-Meals on Wheels items. In the case of malfuncrion, the main office shall be notified immediately and be responsible for the repair. 3. The Auburn site provides volunteers to operate the meal delivery system. 4. The site shall not deny meals to any eligible participant because of his/her ability to donate. In tum, the program will not deny meals to the site because of participanYs inability to pay. The suggested donation per meal is $3.00, and partioiparits must be given the opportunity to de6ide for themselves how much they wish to donafe towazd the cost of the meal. Eligibleparticipants under 60 years ofage must be charged $3.75 for each meal provided to them. 5. All donations received from participanfs shall be handled in accordance with"Meals on Wheels Program Procedures for the Handling of Participant Doriations." It is the responsibility of the Meals on Wheels Coordinator to safeguard the donations. 6. Deposit slips validated by the bank should be forwarded to themain office by month's end along with the weekly deposit reports. 7. The weekly ordering of ineals is the responsibiliry of the Meals on Wheels Site Coordinator: THe order should reflect the nurrtber of clients being served and additional inventory should be kept to a minimum. 8. The participant has the right to select any meal item np to fonrteen meals per person per week. Except in extreme cases, subsUtntions shall not be made without notifying the participant. 9. The program agrees to provide the site with the necessazy forms to assist in the operation of the home- delivered meal service to the eldeily(information sheets, applic.ation forms, menus, etc.). 10. Any program information or commdnity/senior center information intended for Meals on Wheels participants must first be approved by Senior Services of Seattle/King County prior to disiiibution. Exhibit A-Resolution 3418 SeniorCenter Meals on Wheels Page 1 of2 Memorandum of Agreement City of Auburn MOW Satellite Site Page 2 11; Initial assessments to determine eligibility will be made by the Site Coordinator when a pe;son applies to the program: Reassessments of all participants shall be conductedby the Sife Coordinator during January and July of each yeaz and/or on a 6-month basis. 12.The site agrees to subaut Monthly Acrivity Reports to the piograw on the forms provided. 13:The site shall maintain client and other program.service records(meals delivered). Such records shall be maintained for a period of three(3)yeazs. If necessary,any out-dated r�wrds may be stored at our wazehouse. 14.Client data.is considered confidential and the pmperty of Senior Services of SeattleJKing County, Tlris data must not be used for any purpose other than for the Meals on Wheels program. 15. The program shall provide c6nsultation and support servioes. 16.This Memorandum of Agreementmay be termiaated with sixty(60)days written notice by either p.arty. 17.The term of this Agreement shall be&om.January 1, 2002,to December 31, 2002, and may be amended to extend thereafter by mntual agreement of the parties. 18.
