MINUTES
I. CALL TO ORDER
Ghair Jim Kelly called the meeting to order at 10:00 a.m. in Conference Room 2
located on the second floor of Auburn City Hall, 25 West Main Street in Auburn.
Members present included Chair Kelly, Chair Pro Tem Bill Petersen, Member
Yolanda Trout-Manuel, Member Bob Baggett, and Member-at-large David Cook.
Also present were Board Attorney Doug Ruth and Board Secretary Danielle
Daskam.
II. AGEPlDA MODIFICATIONS
An additional claim from LEOFF 1 Member #321061 was added to the agenda
under Item V.C.2.b.

A. Minutes of the March 7 2017 LEOFF Board Meetinq
Motion by Chair Pro Tem Petersen, second by IVlember Trout-Manuel to
approve the minutes as submitted.
MOTION CARRIED UNANIMOUSLY. 5-0
IV. UNFINISHED BUSIIVESS
A. Pendin4 Disabilitv Leave/Retirement Applications
There was no pending disability leavelretirement application.
B. PendinQ Medical/HearinqNision/Dental Claims
There was no pending medical, hearing, vision or dental claim.
V. NEW BUSINESS

No new application for disability leave was received.

Request for approval/reimbursement of �long-term care expenses
for the month of March 2017 in the amount of$5,655.00.
Motion by Member Baggett, second by Chair Pro Tem Petersen to
approve the claim in the amount of$5,655.00.
MOTION CARRIED UNANIMOUSLY. 5-0
2. LEOFF 1 Member#321061
a. Request for reimbursement of dental expenses in the amount of
$163:40 for services received on January 18, 2017. The
LEOFF 1 Member has not received dental expense
reimbursement in 2017.
Motion by Chair Pro Tem Petersen, second by Member Cook to
approve the claim in the amount of$163.40.
MOTION CARRIED UNANIMOUSLY. 5-0
b. Request for reimbursement of dental expenses in the amount of
$293:25 for services received on March 29, 2017.
Motion by Member Cook, second by Chair Pro Tem Petersen to
approve the claim in the amount of$293.25.
MOTION CARRFED UNANIMOUSLY. 5-0
3. LEOFF 1 Member#310533
Request for reimbursement of dental expenses in the amount of
$78.00 for services �eceived on March 6, 2017. The LEOFF 1
Member has not receive dental expense reimbursement in 2017.
Motion by Member Baggett, second by Chair Pro Tem Petersen to
approve fhe claim in the amount of$78:00.

$400.00 for services received February 23 — March 9, 2017.
Motion by Chair Pro Tem Petersen, second by Member Trout-
Manuel to approve the claim in the amount of$400.00.
MOTION CARRLED UNANIMOUSLY. 5-0
5. LEOFF 1 Member#190365
Request for reimbursement of inedical expenses in the amount of
$23.80 for serviees received on January 16, 2017.
Motion by Member Trout-Manuel, second by Member Cook to table
the claim pending the receipt of the IVledicare/insurance
explanation of benefits statement.
MOTION CARRIED UNANIMOUSLY. 5-0
6. LEOFF 1 Member#629199
Request for reimbursement of dental expenses in the amount of
$154:80 for services received on March 29, 2017. The LEOFF 1
member has not received dental reimbursement in 2017.
Motion by Chair Pro Tem Petersen, second by Member Cook to
approve the claim in the amount of$154.80.
MOTION CARRIED UNANIMOUSLY. 5-0
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:08 a.m.
