MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in Conference Room 2
located on the second floor of Auburn City Hall, 25 West Main Street in Auburn.
Members present included Chair Kelly, Chair Pro Tem Bill Petersen, and
Member-at-large David Cook. Members Yolanda Trout-Manuel and Robert
Baggett were excused. Also present were Board Attorney Doug Ruth,and Aeting
Board Secretary Ashley Payment.
II. AGENDA nAODIFICATIONS -
There,was no ehange to the agenda.
III. APPROVAL OF MINUTES
A. Minutes of the June 6, 2017 LEOFF Board Meetinq
Motion by Chair Pro Tem Petersen, second by Member Cook to approve
the minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 3-0
IV. UidFINISHED BUSINESS
A. Pending Disabilitv Leave/Reticement Applications
There was no pending disability leave/retirement application.
, B. Pendinci Medical/Hearing/Vision/Dental Claims

Request for reimbursement of inedical expenses in the amount of
$23.80 for services received on January 16, 2017.
On April 4, May 2, 2017, and June 6 the LEOFF Board tabled the
claim pending receipt of the Medicare/insurance explanation of
benefits statement. No new information was received. Motion by
Chair Pro Tem Peterson, second by Member Cook to deny claim.
MOTION CARRIED UNANIMOUSLY. 3-0

On June 6 the LEOFF Board tabled the claim pending receipt of
the insurance explanation of benefits statement, No new
information was received. Motion by Member Cook, second by
Chair Pro Tem Peterson to table claim until the next meeting.
MOTION GARRIED UNANIMOUSLY. 3-0
V. NEW BUSINESS

C. Medical/HearinQ/Vision/Dental Claims

Request for approval/reimbursement of long-term care expenses
for the month of June 2017 in the amount of$5,965.
Motion by Chair Pro Tem Petersen, second by Member Cook to
approve the claim in the amount of$5,965.
MOTION CARRIED UNANIMOUSLY. 3-0
2. LEOFF 1 Member#562217
Request for reimbursement of vision exam expenses in the amount
of$50.00 for services received on April 19, 2017.
Motion by Chair Pro Tem Petersen, second by Member Cook to
approve fhe claim in the amount of$50.00.
MOTION CARRIED UNANIMOUSLY. 3-0
3. LEOFF 1 Member#642873
Request for reimbursement of vision expenses in the amount of
$179.98. The LEOFF 1 member has not received vision expense
reimbursement in the past year.
Motion by Member Cook, second Chair Pro Tem Petersen to
approved the claim in the amount of$179.98.

4. LEOFF 1 Member#779147
Request for reimbursement of dental expenses in the amount of
$422.75 for services received on June 8, 2017. The LEOFF 1
member received dental expense reimbursement in the amount of
$304.95 on June 6, 2017.
Motion by Member Cook, second by Chair Pro Tem Petersen to
approve the claim in the amount of$422.75.
MOTION CARRIED UNANIMOUSLY. 3-0
5. LEOFF 1 Member#406822
a. Request for reimbursement of vision expenses in the amount of
$190.98 for services received on June 24, 2017. The LEOFF 1
member has not received vision expense reimbursement in the
past year.
Motion by Chair Pro Tem Petersen, second by to approve the
dental claims in the amounts of$190.98.
MOTION CARRIED UNANIMOUSLY. 3-0
b. Request for reimbursement of dental expenses in the amount of
$369.90 for services received on June 5, 2017. The LEOFF 1
member has not received dental expense reimbursement in the �
past year.
Motion by Member Cook; second by Chair Pro Tem Petersen to
approve the dental claims in the amounts of$369.90.
MOTION CARRIED UNANIMOUSLY. 3-0
D. Other Discussion
Chair Pro Tem Petersen discussed a LEOFF member who has not
submitted elaims in years and wants to investigate if the member is still
alive, Chair Kelly stated it may not be within the boards authority. Attorney
Ruth also stated as the fire representative Chair Pro Tem Petersen could
look into this issue. Attorney Ruth offered to give resources to find if there
is a death certificate.
