CALL TO ORDER Chair Jim Kelly called the meeting to order at 10:00 a.m. in Conference Room 2 located on the second floor of Auburn City Hall, 25 Wesi Main Street in Auburn. Members present included Chair Jim Kelly, Chair Pro Tem Bill Petersen, and Member Yolanda Trout-Manuel. Member Bob Baggett arrived at 10:06 a.m. during discussion of Item V.C.S. Member-at-large David Cook was absent: Also present were Board Attorney Doug Ruth and Board Secretary Danielle Daskam. II. AGENDA AAODIFICATIONS Claims for LEOFF 1 member numbers 52515, 349135, 937035, and 310533 were added to the agenda. An additional document was transmitted regarding Item V.C.5 relating to LEOFF 1 member number 629199. III. APPROVAL OF MINUTES A. Minutes of the October 3, 2017 LEOFF Board Meetinp Motion by Member Trout-IVlanuel, second by Chair Pro Tem Petersen to approve the October 3, 2017 minutes as distributed. MOTION GARRIED UNANIMOUSLY. 3-0 B. Minutes of the October 18, 2017 Special LEOFF Board Meetinq Motion by Membe�Trout-Manuel, second by Chair Pro Tem Petersen to approve the minutes as distributed. MOTION CARRIED UNANIMOUSLY. 3-0 IV. UNFINISHED BUSINESS A. Pendinq Disabilitv Leave/Retirement Applications There was no pending disabilif}r leave%retirement application. B. Pending MedicaUHeacing/Vision/Dental Claims 1. LEOFF 1 Member#221843 Request for reimbursement of Medicare expenses in the amount of $3,169,20. On September 5 a.nd October 3, 2017, the Board tabled the claim pending receipt of the Social Security Form 1099 from 2014 and 2015. To date, no additional information has been received. LEOFF 1 Member#221843 Request for reimbursement of inedical expenses in the amount of $222.50 for services received on January 9, 2015 and April 24, 2017. On September 5 and October 3, 2017, the Board tabled the claim pending the receipt of further information, including the insurance/Medicare explanation of benefits statement, and an explanation for the delay in submitting the claim within the policy guidelines. No additional information has been reeeived. Motion by Chair Pro Tem Petersen, second by Member Trout- Manuel to table the claim for an additional month. MOTION CARRIED UIVANIMOUSLY. 3-0 3. LEOFF 1 Member#444135 Request for reimbursement of inedical expenses in the amount of $115.90. On September 5 and Oetober 3, 2017, the Board tabled the claim pending the receipt of further information, including the insurance/Medicare explanation of benefits statement. No new information has been reeeived. Motion by Ghair Pro Tem Petersen, second by Member Trout- Manuel to table the claim for an additional month. MOTION CARRIED UNANIMOUSLY. 3-0 V. NEW BUSINESS A. Motion by Chair Pro Tem Petersen, second by Member Trout- Manuel to approve the claim in the amount of$5,965.00. MOTION CARRIED UNANIMOUSLY. 3-0 - 2. LEOFF 1 Member#363477 a. Request for reimbursement of inedical expenses in the amount of$6.02 for services received on August 10, 2017. b. Request for reimbursement of inedical expenses in the amount of$39.56 for services received on October 4, 2017. Motion by Member Trout-Manuel, seeond by Chair Pro Tem Petersen to approve the claims in the amount of$6.02 and $39.56. MOTION CARRIED UNANIMOUSLY. 3-0 3. LEOFF 1 Member#779147 a. Request for reimbursement of vision expenses in the amount of $189.98 for services received on October 21, 2017. The LEOFF 1 Member has not received vision reimbursement in the past year. Motion by Chair Pro Tem Petersen, second by Member Trout-Manuel to approve the claim in the amount of$189.98. MOTION CARRIED UNANIMOUSLY. 3-0 4. LEOFF 1 Member#970468 Request for reimbursement of dental expenses in the amount of $50.82 for services received on August 22, 2017. The LEOFF 1 member's dental expense was previously determined to be medically necessary. (For clarification of these minutes, this item was discussed and acted on after item V.C.6 of these minutes.j Chair Pro Tem Petersen presided over the meeting during consideration of this item. Motion by Member Trout-Manuel, second by Member Baggett to approve the claim in the amount of$50.82. MOTION CARRIED UNANIMOUSLY. 3-0 Chair Kelly abstained. 5. MOTION CARRIED UNANIMOUSLY. 4-0 Motion by Chair Pro Tem Petersen, second by Member Trout- Manuel to approve the claim in the amount of$1,217.00. MOTION CARRIED UNANIMOUSLY. 4-0 6. LEOFF 1 Member#321061 Request for reimbu�sement of dental expenses in the amount of $163.40 for services received on October 10, 2017, The LEOFF 1 member's dental expense was previously determined to be medically necessary. Motion by Chair Pro Tem Petersen, second by Member Baggetf to approve the claim in the amount of$163.40. MOTION CARRIED UNANIMOUSLY. 4-0 7. MOTION CARRIED UNANIMOUSLY. 4-0 8. LEOFF 1 Member#349135 Request for reimbursement of inedical expenses in the amount of $36.00 for services received March 23, 2017. Motion by Member Baggett, second by Chair Pro Tem Petersen to approve the claim in the amount of$36.00. LEOFF 1 Member#937035 . a. Request for reimbursement in the amount of$263.94 for services received May 2017 to October 2017. Motion by Chair Pro Tem Petersen, second by Member Trout-Manuel to approve the claim for reimbursement of monthly dues at a fitness center. Chair Kelly acknowledged the claim was accompanied by a physician's statement instructing the member to participate in a regular exercise program. Chair Kelly questioned whether instruction to participate in a regular exercise program equates to a membership in a fitness center. Chair Kelly noted the new medical program anticipated to be implemented January 1, 2018 for Medicare eligible members will provide a Silver Sneakers fitness program. Member Baggett stated there are other alternatives for a regular exercise program. MOTION FAILED. 2-2 Chair Kelly and Member Baggett voted no. b. Request for reimbursement of vision expenses in the amount of $219.98 for services received on May 19, 2017. The LEOFF 1 IVlember has not received vision expense reimbursement in the past year. Motion by Member Baggett, second by Chair Pro Tem Petersen to approve vision expenses in the amount of $219.98. MOTION GARRIED UNANIMOUSLY. 4-0 c. Request for reimbursement of vision expenses� in the amount of$139.99 for services received on May 31, 2017. Motion by Member Trout-Manuel, second by Member Baggett to approve the claim in the amount of$139.99. Police LEOFF Representative Election Secretary Daskam reported that nominations for the Police LEOFF Board Representative closed October 19, 2017. One nomination was received, and it was from the current Police LEOFF Board representative, Jim Kelly. In accordance with the LEOFF Policies and Proeedures, Jim Kelly is elected to a new term as Police LEOFF Representative from January 1, 2018 to December 31, 2019. 2. Member-At-Large Resignation The LEOFF Board received a letter of resignation from Member-at- large David Cook. Mr. Cook stated he has moved from the area and outside the city limits of Auburn; therefore, he is no longer eligible to serve on the Board. Mr. Cook thanked the LEOFF Board members for the opportunity to serve on the Board. Board Attorney Ruth reminded that in even numbered years the policies and procedures are to be reviewed for any changes or amendments, He requested members subm�it any recommended amendments to him.
