MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at. 10:00 a.m. in Conference Room 2
located on the second floor of Aubum City Hall, 25 West Main Street in Auburn.
Members present included Chair Jim Kelly, Chair Pro Tem Bill Petersen,
Member Bob Baggett, and Member-at-large Stan Laatsch. Also present were
Board Attorney Doug Ruth and Bo.ard Secretary Danielle Daskam.
II. AGENDA.MODIFICATIONS
A claim from LEOFF 1 member 937035 was added to 4he agenda as Item V.C.6
III. APPROVAL OF MINUTES
A. Minutes of the April 3. 2018-LEOFF_Board-Meetina
Motion by Chair Pro Tem Petersen; second by Member Baggett to
approve the minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 4-0
IV. UNFINISHED BUSINESS
A. Pendinq Disabilitv Leave/Retirement Aaalications
There was no pending disability leave%retirement application.
B. Pendin4 Medical/HearinaNision/Dental Claims

Request for �eimbursement of inedical expenses in the amount of
$44.70 for services received on November 16, 2017.
On Februa_ry 6, March 6 and April 3, 2018, the Board tabled the
claim pending the receipt of the insur"ance explanation of benefts
statement. No new informa4ion was received.
Motion by Chair Pro Tem Petersen, second by Member Baggett to
deny the claim.

Request for reimbursement of inedical expenses in the total
amount of$86.43 for seniices received in 2017.
On March 6 and April 3, 2018, the Board tabled the claim pending
the receipt of the Medicarefinsurance explanation of benefds
statement. No new information was received.
Motion by Chair Pro Tem Petersen, second by Memb.er Bagge.tt to
fable the claim until the next meeting.

Request for reimbursement of inedical expenses in the amount of
$56.59 for services received in November 2017.
On March 6 and April 3, 2018, fhe Board tabled the claim pending
fhe receipt of the Medicarefinsurance explanation of benefits
statement. No new information was received.
Motion by Chair Pro Tem Petersen, second by Member Baggett to
table the claim until the next meeting.
MOTION CARRIED UNANIMOUSLY. 4-0
V. NEW BUSINESS
A. Disabilitv Leave/Retirement Aaalications
No new application for disability leave was received.
B. Appeals/Requests.for Reconsideration
No request for reconsideration was received.
C. Medical/HearinqNision/Dental Claims
1. LEOFF 1 Member#230947
Request for reimbursement derital expenses in the amount of
$160.55 for serVices received on March 29, 2018. The LEOFF
member has not received dental expense reimbursement in 2018.
Motion by Chair Pro Tem Petersen, second by Member Baggett to
approve the claim in the amount of$160.55.

Motion by Chair Pro Tem Petersen, second by member Baggett to
approve the dental expenses in the amount of$2,549.00.

Request for reimbursement of long term care costs in the amount
of$1,776.61 for services received April 1-9, 2018.
Motion by Member Baggett, second by Chair Pro Tem Petersen to
approve the ctaim in the amount of$1,776.61.
MOTION CARRIED UNANIMOUSLY. 4-0
4. LEOFF 1 Member#363477
Request for reimbursement of dental expenses in the amount of
$150.00 for services received on April 11, 2018. The LEOFF
member has not received dental expense reimbursement in 2018.
Motion by Chair Pro Tem Petersen, second by Member Baggett to
approve the claim in 4he amouM of$150.00.
MOTION CARRIED UNANIMOUSLY. 4-0
5. LEOFF 1 Member:#321061
Request for reimbursement of dental expenses in the amount of
$171.00 for senrices received on March 21, 2018. The LEOFF 1
member's dental expenses have previously been determined to be
medically necessary.
Motion by Chair Pro Tem Petersen, second by Member Baggett to
approve the claim in the amount of$171.00.

6. LEOFF 1 Member#937035
Reque.st fo.r reimbursement of dental expenses in the amount of
$405.00 for services received on December 6, 2017. The LEOFF 1
Member did not receive dental reimbursement in 2017.
Motion by Chair Pro Tem Petersen, second by Member Baggett to
approve the claim in the amount of'$405.00.
MOTION CARRIED UNANIMOUSLY. 3-0 Member Laatsch
abstained from voting.
D. Other Discussion
1. LEOFF Board Policies and Procedures
Board Attorney Rufh distributed a draft re.solution adopting changes
to the policies and procedures discussed over the last several
months.
Attorney Ruth stated throughout he removed the reference to
"active" and "retired" and left the term "member" or "members."
The redundant Verbiage on Medicare in Chapters 8.02 and 9.01
was also removed.
The vision benefit was increased to $300.00.
Chair Pro Tem Petersen suggested including dental implants under
dental services. Afterfurther discussion, it was suggested adding a
provision in 8.02(D) stating implants recommended by the treating
physician or dentist will be considered on a case by case and pre-
approval will be required.
Chair Kelly requested the Board Secretary compile information on
the range of costs for dental implants.
