CALL TO 092DER Chair Jim Kelly called the mee#ing to order at 10:00 a.m. in Conferenee Room 2 located on the second floor of Auburn City Hall, 25 West Main Street in Auburn. Members present included Chair Jim Kelly, Chair Pro Tem Bill Petersen, Member Bob Baggett, and Member-at-large Stan Laatsch. Also present were Board Attorney Doug Ruth and Board Secretary Danielle Daskam. II. AGEIVDA iwODIFICATIOIVS There was no change to the agenda. III. APPROVAL OF MIIdUTES A. Minutes of the May 1, 2018 LEOFF Board Meetinq Motion by Chair Pro Tem Petersen, second by Member Laatsch to approve the minutes as distributed. MOTION CARRIED UNANIMOUSLY. 4-0 IV. UNFINISHED BUSINESS A. Pending Disabilitv Leave/Retirement Applications There was no pend'ing disability leave%retirement application. B. Pending Medical/HearingNision/Dental Claims 1. LEOFF 1 Member#398982 Request for reimbursement of inedical expenses in the total amount of$86.43 for services received in 2017. On March 6, April 3 and May 1, 2018, the Board tabled the claim pending the receipt of the Medicare/insurance explanation of benefits statement. No new information was received. Motion by Chair Pro Tem Petersen, second by IVlember Baggett to deny the claim due to lack of information. LEOFF 1 Member#490024 Request for reimbursement of inedical expenses in the amount of $56.59 for services received in November 2017. On March 6, April 3 and May 1, 2018, the Board tabled the claim pending the receipt of the Medicare/insurance explanation of benefits statement. No new information was received. Motion by Chair Pro Tem Petersen, second by Member Baggett to deny the claim due to lack of information. MOTION CARRIED UNANIMOUSLY. 4-0 V. NEW BUSINESS A. Disability Leave/Retirement Applications No new application for disability leave was received, and there are no LEOFF 1 active members. B. Appeals/Requests for Reconsideration IVo request for reconsideration was received. C. Medical/HearinaNision/Dental Claims 1. LEOFF 1 Member#856535 Request for reimbursement medical expenses in the amount of $54.20 for services received on Ocfober 11, 2017. (This is re- submission of a claim previously denied due to lack of Medicare/insurance explanation of benefits statement.) Motion by Chair Pro Tem Petersen, second by Member Baggett to approve fhe claim in the amount of$54.20. MOTION CARRIED UNANIMOUSLY. 4-0 2. LEOFF 1 Member#406822 Request for reimbursement of dental expenses in the amount of $314.10 for services received on May 23, 2018. The LEOFF 1 member has not received dental reimbursement in 2018. , Motion by Member Baggett, second by Chair Pro Tem Petersen to approve fhe claim in the amount of$314.10. LEOFF 1 Membec#64287_3 - - Request for reimbursement of dental expenses in the amount of $214.00 for services received on May 7, 2018. The LEOFF 1 member has not received dental expense reimbursement in 2018. Motion by Chair Pro Tem Petersen, second by Member Baggett to approve the claim in the amount of$214.00. MOTIDN CARRIED UNANIMOUSLY. 4-0 4, LEOFF 1 Member#490024 Request for reimbursement of dental expenses in the amount of $1,500.00 for services received on May 9, 2018. The LEOFF member has not received dental expense reimbursement in 2018. Motion by Chair Pro Tem Petersen, second by Member Baggett to approve reimbursement in the amount of $1,200.00, which represents the annual maximum reimbursement level. MOTION GARRIED UNANIMOUSLY. 4-0 5. LEOFF 1 Member#321061 Request for reimbursement of vision expenses in the amount of $78.00 for services received on April 30, 2018. The LEOFF member received $270 in vision reimbursement on Decembe� 5, 2017. Motion by Chair Pro Tem Petersen, second by Member Laatsch to deny the claim as the member has already received the maximum reimbursement level for the past twelve months. MOTION CARRIED UNANIMOUSLY. 4-0 6. LEOFF 1 Member#779147 Request for reimbursement of dental expenses in the amount of $304.95 for services received on May 3, 2018. The LEOFF member has not received dental expense reimbursement in 2018. Motion by Member Baggett, second by Chair Pro Tem Petersen to approve the claim in the amount of$304.95. LEOFF 1 Member#629199 Request for reimbursemenfi of dental expenses in the amount of $226.00 for services received on May 2, 2018. The LEOFF 1 member's dental expenses have previously been determined to be medically necessary. Motion by Chair Pro Tem Petersen, second by Member Baggett to approve the claim in the amount of$226.00 MOTION CARRIED UNANIMOUSLY. 4-0 D. Other Discussion 1. LEOFF Board Policies and Proced.ur.es Board Attorney Ruth presented and reviewed a resolution to effect policy amendments. The proposed amendments were discussed at the last LEOFF Board meeting. Attorney Ruth explained he added a provision regarding dental implants included with the provisions regarding dentures. He noted the Board has the discretion to inelude reimbursement of implant expenditures in the policies and procedures if the procedure is medically necessary. The Board discussed costs of implants, a requirement for pre- approval, and medical necessity. Attorney Ruth proposed the following language for a new paragraph 5 under Policy 8.02(D): Dental implants may be approved; however, prior authorization by the Board is required. To receive pre-authorization for an implant, the Board first must be provided with a letter from fhe member's treating dentist or physician stating the reason the implant is medically necessary. Chair Pro Tem Petersen moved and Member Baggett seconded to approve Resolution IVo. 2018-01, with the added language regarding dental implants and changing vision benefits to reflect calculation of#he benefit on a calendar year rather than on a rolling 12-month period under Policy 9:02(C). MOTION CARRIED. 4-0 LEOFF Board Minutes June 5, 2018 Page 4 City of Auburn Law Enforcement Officers and Firefighters Disability Board Tuesday, June 5, 2018 - 10:00 AM — Conference Room 2 Chair Kelly inquired regarding the claims process and potential for using electronic signatures for claims. He noted the LEOFF claim form provides for the signature of the LEOFF Fire or Police representative as proof of their review. Chair Kelly recommended removing the Board representative signature from the claim form if it is not required. VI.
