CALL TO ORDER
Chair Kelly called the meeting to order at 10:30 AM in Conference Room 1
located on the first floor of Auburn City Hall, 25 West Main Street, Auburn, WA.
Chair Kelly and the following Board members were present: Chair Pro Tem Bud
Larson, Member Stacey Brothers, Member Gene Cerino, and Member Russ
Vandver. Board Secretary Danielle Daskam was also present.
APPROVAL OF MINUTES
Minutes of the June 2, 2003 LEOFF Board Meetinq
Motion by Chair Pro Tem Larson, second by Member Vandver to approve
the June 2, 2003 minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 5-0.
UNFINISHED BUSINESS
Pending Disability Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Claims
Rudy Peden
Request dated May 6, 2003 for reimbursement of services in the
amount of $86.71.
On June 2, 2003, the Board tabled Mr. Peden's claim pending the
receipt of an insurance explanation of benefits statement and a
physician's statement/letter attesting to the medical necessity for
the medical device. No new information was received.
Motion by Chair Pro Tem Larson, second by Member Cerino to
table Mr. Peden's claim pending-the receipt of the additional
information requested.
MOTION CARRIED UNANIMOUSLY. 5-0.

No new application for disability leave/retirement was received.
B. Appeals
No appeal was filed.
Medical/HearinqNision Claims

Request for reimbursement of frames and lenses expenses dated
April 28, 2003:
Bifocal Lenses $129.48
Frames $169.00
LEOFF Board Policy 9.02(A)(2) provides for reimbursement for
lenses and frames as follows:
Bifocal Lenses $ 60.00
Frames $ 90.00
Board members reviewed Mr. Shropshire's claim.
Motion by Member Cerino, second by Member Brothers to approve
the claim in the amount of $150.00 according to LEOFF policy.
MOTION CARRIED UNANIMOUSLY. 5-0.
Russ Vandver
Claim for payment for reimbursement of medical expenses in the
amount of $120.00 for medical services received on May 2, 2003
and processed by insurance on June 4, 2003.
Board members reviewed Mr. Vandver's claim.
Motion by Chair Pro Tem Larson, second by Member Brothers to
pay the claim as submitted in the amount of $120.00.
MOTION CARRIED. 4-0. Member Vandver abstained from voting.
Robert Reno
Claim for payment for reimbursement of medical expenses in the
amount of $156.38 for medical services received on April 15, 2003.

Chair Kelly noted the enclosed correspondence from LEOFF
Member Russ Vandver to Mr. Reno requesting a completed claim
for payment and explanation of benefits statement.
Motion by Member Cerino, second by Chair Pro Tem Larson to
table Mr. Reno's claim pending the receipt of a complete claim
including an explanation of benefits from his insurance and
information on Mr. Reno's condition, treatment and prognosis.
MOTION CARRIED UNANIMOUSLY. 5-0.
Stan Laatsch
Request for reimbursement of eye exam and
dated June 26, 2003:

Exam $ 25.00
Progressive $ 97.00
LEOFF Board Policy 9.02(A)(1) provides for reimbursement for
lenses as follows:
Progressive $100.00
Motion by Chair Pro Tem Larson, second by Member Vandver to
approve payment in the amount of $122.00.
MOTION CARRIED UNANIMOUSLY. 5-0.
Coy F. Grail
Claim for payment for reimbursement of medical expenses in the
amount of $90.00 for medical services received on April 15, 2003
and processed by insurance on June 2, 2003.
(Mr. Grail's claim was added as an agenda modification.)
Board members reviewed Mr. Grail's claim.
Motion by Member Vandver, second by Chair Pro Tem Larson to
approve Mr. Gralrs claim in the amount of $90.00.
MOTION CARRIED UNANIMOUSLY. 5-0.

D. Correspondence
Letter dated June 16, 2003 addressed to Alan Kosko from Jim
Kelly, LEOFF Board Chair.
Chair Kelly noted his letter to Mr. Kosko in response to Mr. Kosko's
May 27, 2003 letter regarding dental benefits for retirees and
medical insurance coverage for spouses.

There being no further business to come before
adjourned at 10:50 AM.
