MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in Conference Room 2
located on the second floor of Auburn City Hall, 25 West Main Street in Auburn.
Members present included Chair Jim Kelly, Chair Pro Tern Bill Petersen,
Member Bob Baggett, Member Larry Brown and Member-at-large Stan Laatsch.
Also present were Board Attorney Steve Gross and City Clerk Shawn Campbell.
II. AGENDA MODIFICATIONS
Claims for LEOFF 1 Members 406822 and 321061 were added to the agenda
under Items V.C.3. and V.C.4 respectively.
III. APPROVAL OF MINUTES
A. Minutes of the September 4, 2018 LEOFF Board Meeting
Motion by Chair Pro Tem Petersen, second by Member Laatsch to
approve the minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 5-0
IV. UNFINISHED BUSINESS
A. Pending Disability Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Pending Medical/Hearing/Vision/Dental Claims
There was no pending claim.
V. NEW BUSINESS
A. Disability Leave/Retirement Applications
No new application for disability leave was received.
B. Appeals/Requests for Reconsideration
No request for reconsideration was received.

a. Request for reimbursement of dental expenses in the
amount of $95.00 for services received on August 28, 2018.
The LEOFF 1 member has not received dental
reimbursement in 2018.
Motion by Chair Pro Tem Petersen, second by Member
Laastch to approve the claim in the amount of$95.00.

2. LEOFF 1 Member#910342*
a. Request for reimbursement of dental expenses in the
amount of$67.83 for services received on March 20, 2018.
b. Request for reimbursement of dental expenses in the
amount of$126.45 for services received on June 12, 2018.
c. Request for reimbursement of dental expenses in the
amount of$85.75 for services received on July 26, 2018.

Motion by Chair Pro Tem Petersen, second by Member
Baggett to approve the claims a, b and c in the amount of

Request for reimbursement of medical expenses in the amount of
$162.88 for services received on April 12, 2018.
Motion by Chair Pro Tern Petersen, second by Member Baggett to
approve the claim in the amount of$162.88.

4. LEOFF 1 Member#321061*
Request for reimbursement of dental expenses in the amount of
$240.80 for services received on September 24, 2018. The retirees
dental expenses have been previously found to be medically
necessary
Motion by Chair Pro Tern Petersen, second by Member Laatsch to
approve the claim in the amount of$240.80.
MOTION CARRIED UNANIMOUSLY. 5-0

There was no other discussion.
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:11 a.m.
