MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in Conference Room 2
located on the second floor of Auburn City Hall, 25 West. Main Street in Auburn.
Members present included Chair Jim Kelly, Chair Pro Tern Bill Petersen,
Member Bob Baggett, Member Larry Brown and Member-at-large Stan Laatsch.
Also present were Board Attorney Doug Ruth and Deputy City Clerk Teresa
Mattingly.
II. AGENDA MODIFICATIONS
Claims for LEOFF 1 Members 215086 was added to the agenda under Items
V.C.3.
III. APPROVAL OF MINUTES
A. Minutes of the August 7, 2018 LEOFF Board_Meeting
Motion by Chair Pro Tern Petersen, second by Member Baggett to
approve the minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 5-0
IV. UNFINISHED BUSINESS
A. Pending Disability Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Pending Medical/HearingNision/Dental Claims
There was no pending claim.
V. NEW BUSINESS
A. Disability Leave/Retirement Applications
No new application for disability leave was received.
B. Appeals/Requests for Reconsideration
No request for reconsideration was received.

a. Request for reimbursement of vision expenses in the
amount of $194.98 for services received on July 12, 2018.
The retiree has not received vision expense reimbursement
in 2018.
Motion by Chair Pro Tem Petersen, second by Member
Baggett to approve the claim in the amount of $194.98.
MOTION CARRIED UNANIMOUSLY. 5-0
b. Request for reimbursement of dental expenses in the
amount of$109.00 for services received on August 7, 2018.

Motion by Chair Pro Tern Petersen, second by Member
Baggett to approve the claim in the amount of$109.00.
MOTION CARRIED UNANIMOUSLY. 5-0
2. LEOFF 1 Member#190365*
a. Request for reimbursement of dental expenses in the
amount of $61.00 for services received on July 9, 2018. The
retiree's dental expenses were previously found to be
medically necessary. (This claim was denied on August 7th
due to incorrect claim documentation.)
Motion by Member Baggett, second by Chair Pro Tern
Petersen to approve the claim in the amount of$61.00.
MOTION CARRIED UNANIMOUSLY. 5-0
b. Request for reimbursement of dental expenses in the
amount of $175.75 for services received on August 15,
2018. The retiree's dental expenses were previously found
to be medically necessary.

Motion by Chair Pro Tern Petersen, second by Member
Baggett to approve the claim in the amount of $175.75.
MOTION CARRIED UNANIMOUSLY. 5-0
3. LEOFF 1 Member#215086*
Request for reimbursement of vision expenses in the amount of
$254.98 for services received on August 24, 2018. The LEOFF 1
has not received vision expense reimbursement in the past year.
Motion by Member Brown, second by Chair Pro Tern Petersen to
approve the claim in the amount of$254.98.
MOTION CARRIED UNANIMOUSLY. 5-0
D. Other Discussion
There was no other discussion.
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:04 a.m.
